Job summary
We are seeking a B6 Frailty Nurse to join our Assessment and Care Team. Working at a PCN level, the AC Team support frail, housebound patients and care home residents registered at our four GP surgeries. This post is for 22.5 hours a week.
Main duties of the job
Supporting patients to live safely and independently at home.
Reducing admissions to hospitals.
Weekly calls to Care Homes for reviews of residents.
Making referrals to external agencies eg Adult Social Care and voluntary sector services.
Annual reviews and long term condition monitoring.
Vaccinations including flu, covid, shingles, pneumonia and RSV.
Care planning and treatment plans.
Performing health assessments in a community setting (usually the patient home)

Central Bournemouth Primary Care Network is a PCN with four GP surgeries and 50k registered patients.
You will be joining an established team with a wealth of experience supporting our frail and care home population.
This post is for 22.5 hours a week which could be worked as school friendly hours across five days or as three standard days.
We offer 28 days annual leave plus bank holidays (pro rata for part time roles).
