Job summary

We are looking for a compassionate, adaptable and motivated nurse to join our team supporting people experiencing homelessness, insecure housing, substance misuse and other forms of severe disadvantage.

This is not a traditional nursing role. You will combine practice-based nursing with structured outreach, working alongside healthcare professionals, housing providers, recovery services and community organisations to improve access to healthcare for people who often face significant barriers to engagement.

You will support a wide range of clinical activities including wound care, long-term condition management, preventative healthcare, harm reduction and health promotion. You will also help develop new ways of working that improve health outcomes and reduce inequalities.

The role offers variety, autonomy and the opportunity to build meaningful relationships with patients over time. You will be part of a supportive multidisciplinary team that values collaboration, innovation and learning.

Previous inclusion health experience is welcome but not essential. We are looking for someone with strong nursing skills, a willingness to learn and a commitment to compassionate, person-centred care.

Main duties of the job

The successful candidate will divide their time between delivering clinical care within our specialist primary care service and supporting structured outreach activities across Exeter. Working closely with patients, partner organisations and the wider multidisciplinary team, you will help improve access to healthcare for people who often face significant barriers to engagement.

The role includes a mixture of direct patient care, preventative health work, long-term condition support, outreach activity and collaborative working with housing, recovery and voluntary sector services. You will have the opportunity to build trusted relationships with patients, support people to access the care they need and contribute to the ongoing development of innovative inclusion health services.

Job description Job responsibilities Job Purpose

To provide high-quality nursing care and structured outreach support to people experiencing homelessness, insecure housing, substance misuse, social exclusion and other forms of severe disadvantage.

The postholder will work across both practice-based and community settings, helping improve access to healthcare, support long-term condition management, deliver preventative healthcare interventions and build trusted relationships with patients who may not engage with traditional healthcare services.

The role will contribute to the development of Inclusion Health Devons outreach model and work closely with partner organisations including the Exeter STaR programme to deliver coordinated, person-centred care.

Key Duties and ResponsibilitiesClinical CareDeliver safe, effective and compassionate nursing care within professional competence and NMC standards.Undertake clinical assessment, treatment and monitoring of patients attending the service.Deliver wound care, dressings, injections and other nursing procedures as appropriate.Support the management of acute and ongoing health concerns within agreed protocols.Maintain accurate and contemporaneous clinical records using SystmOne and other approved systems.Long-Term Conditions and Preventative CareSupport patients with long-term conditions including diabetes, respiratory disease and cardiovascular disease.Contribute to preventative healthcare programmes including immunisations, screening programmes and health promotion activities.Support recall and engagement initiatives designed to improve health outcomes for vulnerable populations.Promote healthy lifestyles and self-management wherever appropriate.Outreach and EngagementDeliver structured outreach activity in community settings alongside partner organisations.Support patients who may experience barriers to accessing traditional healthcare services.Build trusted therapeutic relationships with patients experiencing homelessness, substance misuse, mental health difficulties and social exclusion.Undertake planned outreach activity linked to multidisciplinary team discussions and agreed care plans.Assist patients to access healthcare, housing, recovery and support services.Substance Misuse and Harm ReductionSupport patients receiving treatment for substance misuse within agreed clinical pathways.Administer Buvidal and other treatments within competency and organisational protocols.Deliver harm reduction interventions and health promotion.Support blood-borne virus screening and associated follow-up pathways.Work collaboratively with recovery and treatment services.Partnership WorkingParticipate in multidisciplinary team meetings including STaR MDTs and other partner forums.Work collaboratively with healthcare professionals, housing teams, social care providers and voluntary sector organisations.Contribute to coordinated care planning and information sharing within governance frameworks.Safeguarding and GovernanceMaintain awareness of safeguarding responsibilities and act appropriately where concerns arise.Follow organisational policies, clinical governance procedures and professional standards.Participate in audits, quality improvement activities and service development initiatives.Support incident reporting, learning and reflective practice.Service DevelopmentContribute ideas and feedback to support ongoing service improvement.Assist in developing innovative approaches to outreach, prevention and inclusion health care.Participate in training, supervision and professional development activities.

Person Specification

Experience Essential

NMC Registered Nurse Experience in primary care, community nursing or a comparable autonomous clinical environment Experience in wound care, including complex wound management and dressing clinics Ability to work independently and safely whilst contributing to a wider multidisciplinary team Strong communication and relationship-building skills Commitment to safeguarding, professional standards and reducing health inequalities Compassionate, trauma-informed approach to patient care

Desirable

Experience working with people experiencing homelessness, substance misuse, severe mental illness, complex trauma or other forms of social exclusion Experience supporting long-term condition management including diabetes, respiratory disease or cardiovascular disease Experience of outreach, assertive engagement or service navigation Experience of OST services, harm reduction or BBV screening Experience using SystmOne, Halo or similar clinical systems
