Job summary
An excellent opportunity has become available for a highly motivated, flexible and passionate Advanced Clinical Practitioner to join the Older Adult Home Treatment teams. People currently undergoing ACP training are eligible to apply for Band 7 and people who have completed the training are eligible to apply for Band 8a. Applicants must hold an ACP qualification or be currently undertaking an ACP programme to be considered for this post.
Currently there are two Home Treatment Team within the Older Adult Service which the post holder will work across:
Croydon Home Treatment Team based at the Bethlem Royal Hospital Lambeth,Southwark and Lewisham (LSL) Home Treatment Team based at the Maudsley Hospital
The post holder will support the clinical team leader in providing clinical leadership and managerial oversight for the allocated caseload. They will support transfers across the Older Adult pathways taking a lead in clinical decision making
Please note that this post is for a 6 month Fixed Term contract however, 6 month Secondment will also be considered.
Main duties of the job
The post holder will deliver, and lead on the delivery of, high quality safe and contemporary care with the whole Older Adult Home Treatment service, ensuring consistently high standards of practice and clinical excellence, including: promoting population health, improving personal outcomes, contributing towards service evaluation, while leading and developing research and quality improvement initiatives.

Person Specification
QUALIFICATIONS & TRAINING Essential
First level professional nursing /AHP/Social Work qualification. Post-registration experience and/or evidence of formal courses and training at post graduate diploma level
Desirable
Quality Improvement Methodology Training, e.g. LEAN Recognised Management or Leadership Qualification
Experience Essential
Extensive experience of leading the delivery of care for a diverse population Advanced level understanding of specialist-specific disorders and their corresponding evidence-based interventions Contemporary knowledge of relevant legislation and healthcare policies Applied understanding of issues relating to confidentiality and mental capacity Applied knowledge of safeguarding, inclusion and equality Experience working within healthcare services at an advanced level of practice
