Practice Nurse (28 hours per week)

Newent Doctors Practice has an exciting opportunity for a practice nurse to join our expanding practice in semi-rural Newent. Newent is a small market town which is central for a Gloucester, Ross, Malvern, Ledbury, or a Tewkesbury commute.
We are looking for an experienced practice nurse but are also willing to train the right nurse for a long-term role.
We are offering a position of 28 hours per week across four days.
Salary is negotiable depending on qualifications and experience.
Please note you will be expected to support our enhanced access clinics and work a late shift every Thursday evening as part of your weekly contractual hours and approximately 3 Saturday mornings a year for which you will be paid overtime.

Main duties of the job
The successful candidate will join our nursing team and will be working alongside our nurse lead, practice nurses, healthcare assistants and phlebotomists to provide a range of nursing services to our patients.
Ideally the post holder is an experienced practice nurse who, acting within their professional boundaries will be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.
They will work as a senior member of the practice multidisciplinary team and will ensure nursing services are delivered effectively to the entitled patient population.

Main duties of the jobThe core responsibilities of the Practice Nurse are as follows. On occasion, there may be a requirement to carry out tasks not listed below, that are within the reasonable scope of the role.
To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy
To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence based practice
Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
Provide routine nursing care to patients in accordance with clinical based evidence, NICE and NSF
To diagnose and manage acute and chronic conditions as required
To review patient medications to enhance compliance
Co-ordinate the provision of travel medicine services
Maintain accurate clinical records in conjunction with existing legislation
Ensure read codes are used effectively by all member of the nursing team
Ensure appropriate use of the chronic disease registers
Prioritise health issues and intervene appropriately
To support the team in dealing with clinical emergencies
Recognise, assess and refer patients presenting with mental health needs
To implement vaccination programmes for adults and children

Experience Essential
Experience of working within a nursing team Experience of working as a practice nurse or community nurse Experience of chronic disease management Wound care/removal of sutures and staples ECGs Venepuncture New patient medicals Requesting pathology tests and processing results Understanding requirement of PGDs and associated policies Women's health (cytology, contraception, etc) Immunisations (routine, childhood and travel) Chronic disease management (Diabetes, Asthma, COPD, etc) Travel Medicine Spirometry Experience in infection prevention and control Ability to record accurate notes
Desirable
