We are looking for an experienced Advanced Nurse Practitioner (ANP) to join our friendly and professional team in serving our patients.
The successful candidate will be an experienced, motivated and enthusiastic ANP.
The post is for 3 days per week (one of the days must be a Tuesday).
Main duties of the job
Responsible for the clinical assessment, investigation, diagnosis and treatment and/or referral of patients within urgent care services, identifying complex and specialist needs and will develop, implement and evaluate programmes/plans of care.
Working as an ANP with a high degree of autonomy, the post holder will manage undifferentiated and undiagnosed minor injuries and minor illness, using advanced clinical skills within own sphere of competence

PURPOSE OF ROLE
Responsible for the clinical assessment, investigation, diagnosis and treatment and/or referral of patients within urgent care services, identifying complex and specialist needs and will develop, implement and evaluate programmes/plans of care.
Working as an Emergency Care Practitioner with a high degree of autonomy, the post holder will manage undifferentiated and undiagnosed minor injuries and minor illness, using advanced clinical skills within own sphere of competence.
Providing direct consultations which dependent upon competency and area of practice, may be via telephone or face to face.
The post holder will work collaboratively with the team and external providers, to ensure the right care, by the right person, at the right time and in the right place. Practitioners will aim to enable individuals to maintain or regain independence or ensure a plan is in place for them to achieve their optimum personal potential and to prevent unnecessary hospital admission.
The post holder will demonstrate a commitment to providing a quality, evidence based service, excellence in clinical practice, this together with continuous professional development are integral to the role and development of the service.

MINIMUM QUALIFICATIONS AND EXPERIENCE REQUIRED FOR THIS POST
Recognised professional degree or equivalent Postgraduate study at Masters level Registered with appropriate professional regulatory body Relevant post registration qualification in autonomous practice or equivalent. Evidence of relevant post registration experience Independent Prescribing or evidence of working to PGDs with advanced knowledge (AHPs) Evidence of continuing professional development Excellent written and verbal communication skills Computer literacy Knowledge of primary and urgent care issues

The ANP is expected to:
The post holder will take a lead role in the advanced clinical assessment, investigation, diagnosis, treatment and /or referral/discharge of a diverse and undifferentiated and undiagnosed problems, in accordance with professional standards and codes of conduct, national best practice and locally and nationally agreed standards. Will act as a credible expert, leading a team in the care of patients, who may have highly complex and or/ chronic presentation, developing, evidencing high-level assessment, assimilation of information implementing and evaluating personalised clinical management plans. Undertaking appropriate investigations to formulate a diagnosis implement and evaluate treatment/ interventions and care for patients presenting with an undifferentiated diagnosis, complex needs or long term conditions. Prescribing within the boundaries of extended non-medical prescribing The post holder will provide critical thinking and analytical skills in practice, and attainment of knowledge, and use their experience to support practitioners in the development of their own skills and knowledge. To act as a role model, resource and mentor, promoting excellence in practice, maintaining and monitoring standards of care, highlighting and working with others as appropriate to manage performance where there is cause for concern. Both providing and participate in clinical supervision and undertaking peer review of practice. Support the collation of the training needs analysis, promoting learning and creating a positive learning environment. Participate in quality improvement, supports the development of service improvements, and also utilises knowledge of clinical governance to provide safe and effective services to individuals. Provides support, advice, guidance and training to patients, families and carers, to promote the understanding and consent to care. Access's research and interprets research, identifies gaps in practice, utilises appropriate evidence bases to support practice development and applies research evidence into working practice.

Person Specification
Experience Essential
Evidence of recent and on-going personal development including obvious leadership development Experience of education and training issues/requirements for undergraduate and post graduate members of professions. Extensive post registration experience in relevant urgent or emergency care settings Skills for analysis of service delivery and improvement planning.

Able to demonstrate a working knowledge and experience of the governance agenda locally.
Qualifications Essential
Registered Health Care Professional holding current registration with appropriate regulatory body. (Nurse or Paramedic) Postgraduate study at Masters level Educated to degree level Relevant post registration qualification in autonomous practice Non Medical Prescribing qualification or evidence of working to PGDs with advanced knowledge (paramedic) Safeguarding-level 3 or willingness to Undertake Teaching and assessing qualification
