Job summary <p style="margin:0px;font-size:inherit;padding:5px 0px;">Are you a motivated and compassionate Registered Nurse with a passion for delivering high-quality care in the community?</p><p style="margin:0px;font-size:inherit;padding:5px 0px;">We are looking for an experienced Band 6 Team Sister / Charge Nurse to join our Community Nursing team supporting patients across the Sheppey locality. This is an exciting opportunity to take on a senior clinical role within a supportive multidisciplinary team, delivering care to patients within their own homes and community settings.</p><p style="margin:0px;font-size:inherit;padding:5px 0px;">You will work closely with the Team Lead and wider community teams to support the day-to-day coordination of services while delivering and overseeing high standards of patient-centred care.</p><p style="margin:0px;font-size:inherit;padding:5px 0px;">If you are forward-thinking, organised and committed to delivering outstanding care within the community, this could be the role for you.</p>
Main duties of the job <p style="margin:0px;font-size:inherit;padding:5px 0px;">As a Band 6 Team Sister / Charge Nurse, you will:</p><ul style="margin-top:0px;margin-bottom:10px;"><li style="font-size:inherit;">Support the Team Lead with the day-to-day coordination of the community nursing service</li><li style="font-size:inherit;">Provide clinical leadership and act as a role model to the wider nursing team</li><li style="font-size:inherit;">Undertake holistic assessments and deliver evidence-based nursing care within patients homes and community settings</li><li style="font-size:inherit;">Coordinate care plans and ensure continuity of care for patients with complex health needs</li><li style="font-size:inherit;">Promote patient independence and support rehabilitation and recovery goals</li><li style="font-size:inherit;">Work closely with the multidisciplinary team including GPs, therapists and social care colleagues</li><li style="font-size:inherit;">Ensure a safe and effective service through appropriate clinical governance and risk management</li><li style="font-size:inherit;">Maintain accurate and contemporaneous patient records</li><li style="font-size:inherit;">Communicate effectively with patients, carers and the wider healthcare team</li><li style=&quo
