Job summary
We have an exciting opportunity for a Clinical Pharmacist to join our PCN which covers 3 practices in Redbridge.
We are looking for an experienced Clinical Pharmacist who is a prescriber and has completed their CPPE course to join our current team . The role is a full-time role, working across the three practices with face to face and telephone consultations in each of the practices.
Main duties of the job
As a Clinical Pharmacist within the PCN, you will work as part of the practice team, supporting GPs, nurses, and other healthcare professionals to deliver high-quality, safe, and effective prescribing. You will be responsible for medicines management, structured medication reviews (SMRs), and improving patient outcomes through proactive, evidence-based pharmaceutical care. You will also be a non-medical prescriber and contribute to chronic disease management, supporting patients with polypharmacy and deprescribing where appropriate.

Consisting of 3 practices, all geographically close and based in Ilford, we have a population of circa 33,000 patients. We feel we are in tune with the need to recognise and work effectively with the concept of population health management and recognise the importance of the newly emerging primary care roles in this context. We are conscious of the need to change current traditional ways of working and begin to benefit from the concept of the patient seeing the right healthcare professional to get the most appropriate support and guidance. Nowhere is this more relevant than in the area of medication usage where significant gains can be made by introducing pharmacist leadership into any strategies the PCN will pursue in this area of care.
Job description Job responsibilities
We are looking for someone, who can act within their professional boundaries, supporting and working alongside a team of Health Care Practitioners within general practice and a Primary Care Network (PCN). The post holder will provide primary support to general practice staff across the PCN, with regards to prescription and medication queries. They will perform medicines reconciliation on transfer of care and implement systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the PCN. The post holder will work as part of a multi-disciplinary team in a patient-facing role. They will take responsibility for areas of medicines optimisation within the PCN and undertake structured clinical medication reviews to proactively manage patients with complex polypharmacy. The post holder will provide clinical leadership on medicines optimisation and quality improvement (QI) and manage some aspects of the quality and outcomes framework and enhanced services at practice and PCN level. The post holder will work with all sectors of pharmacy across the health system to improve population health. Demonstrating improvement in patient outcomes and working to reduce health inequalities across the PCN contributing to improving the efficiency of general practice and delivering a best in class service, the post holder will be supported to develop their role.
