The Hospital Discharge Team are based at the Diana Princess of Wales Hospital. The Hospital Discharge Team is responsible for the safe and effective discharge of patients from hospital into the community and also the prevention of admission to hospital.

The Hospital Discharge Team works in partnership with the Acute Trust ensuring effective prevention and discharge pathways are followed. The postholder will provide professional support to those staff working in the Hospital Discharge Team.

The team operates 7 days per week working between the hours of 8.00am and 8.00pm.

Main duties of the job

Hours: Part-time, permanent (up to 22.5 hours per week available) Less hours will be considered

Salary: Range 3 point 12 £24,968 Pro Rata Per Annum (Fixed Point)

You will be expected to work as part of the HDT team under the direction of the Clinical Lead, to ensure the smooth flow of patients through the team. It is expected that by coordinating administrative tasks, and liaising with other departments, you will contribute to the efficient and effective running of the Discharge Team resulting in a significant reduction in patient journey times and an effective discharge plan being in place.

Person Specification

Experience Essential

Minimum of 6 months administration/reception experience. Experience of data input, spread sheets/statistics. Competent in Microsoft office including Word, Outlook & Excel Stock control Diary management Servicing meetings

Desirable

SystemOne Experience of working with people with varying levels of communication skills/abilities Experience of working in a care environment Supervisory experience

Knowledge & Skills Essential

Ability to effectively manage own workload. Accurate data input and word processing. Accurate minute taking. Effective interpersonal and communication skills, both face to face and over the phone. Ability to liaise effectively with people at all levels. Effective customer care skills. Ability to maintain confidentiality. Ability to demonstrate empathy and sensitivity when dealing with clients/patients.

Qualifications Essential

Literacy and numeracy level 2 qualification Level 3 or equivalent qualification in computer applications Level 2 NVQ Business Administration

Desirable

Business Administration level 3

Other Essential

Ability to demonstrate commitment to CPG core values Ability to work flexibly regarding working hours including weekends/arrangements and venues
