Communicate any changes in a child or young person's condition promptly to the CCN and wider multidisciplinary team.

Collaborate effectively with all members of the multidisciplinary team to ensure coordinated and seamless care.

Utilise an empowerment and family-centred approach to care, supporting families to be active partners and contributing to effective transition planning into adult services.

Provide a safe alternative to hospital admission and support early discharge when appropriate, ensuring robust risk assessment and risk management are consistently applied.

Promote the safeguarding and protection of children and young people by identifying and escalating concerns in line with Local Safeguarding Children Board and Trust policies and procedures.

Participate in clinical supervision and safeguarding supervision in accordance with Trust policy.

Demonstrate commitment to Improving Working Lives (IWL) principles and work flexibly to meet the needs of the service.
