Job summary

We are looking for an enthusiastic and motivated GP to join our thriving and innovative team at Millway Medical Practice. We are advertising for up to 4 clinical sessions for a GP ARRS session. The GP will need to be present in the practice (this is not a remote role). The days of the week can be negotiated but ideally be available on a Thursday.

Main duties of the job

Provide high-quality, safe and patient-centred general medical services to a diverse practice population.

Undertake face-to-face, telephone and digital consultations, assessing, diagnosing and managing patients presenting with a wide range of acute and chronic health conditions.

Work collaboratively within a multidisciplinary team, including pharmacists, first contact practitioners, mental health practitioners, social prescribers, care coordinators and nursing staff, to deliver integrated and personalised care.

Support the effective delivery of the practice's total triage model, ensuring patients are directed to the right clinician or service at the right time.

Contribute to the proactive management of long-term conditions, preventative healthcare, screening programmes and health promotion initiatives.

Participate in the review and management of complex patients, including those with frailty, multiple long-term conditions and mental health needs.

Support quality improvement activities, clinical audits, service development and implementation of new models of care aligned with Primary Care Network and NHS priorities.

Maintain accurate and contemporaneous clinical records using the practice clinical system and work in accordance with GMC standards, national guidance and local policies.

Contribute to practice meetings, multidisciplinary discussions and shared learning activities to support continuous improvement in patient care.

Job description Job responsibilities The post-holder will provide high-quality, patient-centred primary care services as part of a multidisciplinary team, delivering safe, effective and evidence-based care to the practice population.

Key responsibilities include:

Making professional, autonomous clinical decisions in relation to presenting problems, whether self-referred or referred by other healthcare professionals.

Assessing, diagnosing and managing patients presenting with undifferentiated and undiagnosed conditions across a wide range of clinical settings.

Undertaking face-to-face, telephone and digital consultations in accordance with practice protocols and national guidance.

Identifying disease risk factors, promoting early diagnosis and supporting preventative healthcare initiatives.

Developing and implementing personalised care and treatment plans in partnership with patients, carers and other healthcare professionals.

Providing health education, self-management support and lifestyle advice to improve patient outcomes and wellbeing.

Managing acute and chronic disease presentations in line with current clinical guidelines and best practice.

Referring patients appropriately to secondary care, community services, PCN colleagues and other healthcare providers where required.

Working closely with pharmacists, nurses, mental health practitioners, social prescribers, care coordinators and other multidisciplinary team members to deliver integrated care.

Maintaining accurate, clear and contemporaneous clinical records in accordance with GMC standards, information governance requirements and practice policies.

Prescribing safely, effectively and cost-effectively in accordance with local and national prescribing guidance.

Participating in clinical audit, quality improvement activities, significant event reviews and service development initiatives.

Where possible, attending and participating in practice meetings, multidisciplinary team meetings and educational sessions.

Qualifications Essential

MBBS (or equivalent) MRCGP (or equivalent)
