Job summary
We are looking for a compassionate and motivated Practice Nurse with an interest in frailty to join our friendly and supportive primary care team.
This is an exciting opportunity for an experienced nurse who is passionate about improving outcomes for all patients including older adults and helping patients maintain independence, wellbeing, and quality of life within the community.
Main duties of the job
The successful candidate will work collaboratively within our multidisciplinary team to deliver high-quality nursing care across the practice population, with a particular focus on patients living with frailty and complex long-term conditions.
The role will include:
Chronic disease management Health promotion and preventative care Frailty assessments and care planning Supporting care home residents and housebound patients Medication reviews and monitoring Vaccinations, wound care, and routine nursing duties Working closely with GPs, pharmacists, paramedics, social prescribers, and community teamsWhat We Are Looking For
We are seeking a nurse who:
Is a registered nurse with current NMC registration Has experience in primary care or community nursing Has an interest in frailty, elderly care, or complex care management Demonstrates excellent communication and teamwork skills Is compassionate, proactive, and patient-centred Ideally has experience with long-term condition management and care planningAdditional qualifications in frailty, gerontology, or long-term condition management would be advantageous but are not essential.

About us
We currently care for more than 13,000 patients and are a semi-rural practice.
The partnership is made up of 3 GP partners, supported by 7 salaried GPs. The practice and its patients along with the relationships of the clinical and administrative teams are long established and the practice has consistently achieved high levels of QOF targets for several years and is focused on continuous quality improvement.
We are a forward thinking practice keen to develop not only the service we provide to our patients, but to develop our teams and staff to be the best that they can be. The partners recognise how important it is not only to deliver excellent patient care but to also take care of the whole team with their collaborative approach.
