The service is looking for a band 4 Assistant Practitioner to fill a 22.5 hour vacancy. We are looking for a motivated, reliable and kind practitioner to join the team.

The continence team is small and friendly, working closely with the PCH teams and the Urology department at the JPUH.

The successful candidate will be required to work across the East Coast Community Healthcare area. Working in clinics, care homes and the community.

You will be required to work with both adults and children with continence needs carrying out a comprehensive initial and review assessments, developing treatment and management plans. Changing routine catheters will be a part of the role. You will deliver training and also give help and advice to professionals. Experience in community working is preferable. Knowledge of continence care would be an advantage and we will help you to develop your skills and knowledge in this specialist area. The role is for 22.5 hours a week. The team do not work weekends or bank holidays.

Main duties of the job

Support the continence team to provide continence promotion, treatment strategies and management options to patients with continence problems. This will be delivered through carrying out continence assessments and reassessments in the community, care home, clinic and community hospital setting.

Develop and maintain close links with other professionals.

The Assistant Practitioner will provide care to patients with continence issues. They will work alongside a registered nurse and allied health professionals.

The Assistant practitioner will be indirectly supervised by a registered nurse.

They will be required to provide education to others regarding continence care. This will be achieved through clinical practice, delivering teaching and providing advice and support.

Professional:

*Support the continence nursing team to provide continence advice and support to patients, their carers/relatives and other health care professionals.

*Plan and support care taking into account own competencies.

*Review the effectiveness of advice given and where appropriate initiate any action that may be required.

*Carry out continence assessments, reassessments, promotion and management within care homes, the clinic and in the community setting.

*Promote confidence in the patient by acting appropriately to the organisation's standards of appearance, behaviour and attitude.

*Work in a team and alongside other agencies.

*Safeguard the confidentiality of all information in line with organisational policies.

*Communicate effectively with patients, relatives and colleagues at all times to promote high standards of healthcare.

*Support and participate in clinical audit.

*Carry out routine catheter changes.

Educational:

*Demonstrate a positive attitude to education and participate in continuing professional development in accordance with the organisation's policy.

*Act as a role model for health care professionals and students/learners providing advice and support.

*Participate in teaching in the clinical setting and group teaching.

*Participate in staff induction programme.

*Participate in multi agency work/training.

*Initiate and maintain resource files using evidence based information.

*Attend in-service training courses and study sessions as required.

*To participate in the training of learners by demonstrating procedures

*Taking part in clinical supervision sessions.

Organisational:

*Maintain effective liaison with the continence team, and other health care professionals.

*Comply with the provisions of the Organisation's policies on Health and Safety at work and risk assessment. Reporting hazards within the working environment and completing DATIX documentation as necessary, in accordance with the Organisation's policies.

*Maintain all items of equipment issued to assist in the care of patients including uniform, reporting faults and deficiencies promptly and ensuring that repairs are carried out.

*Ensure the accurate completion of records, care plans and patient registers within agreed Organisational standards.

*Submit all personal claim forms.

Person Specification

Qualifications Essential

*Foundation Degree in Health-related area (or equivalent i.e. Higher Apprenticeship, NVQ level 3 with relevant experience and evidence of further training). *Evidence of personal development.

Desirable

*Evidence of relevant course attendance or experience i.e. Continence Link Care, attended Continence Study Day.

Experience Essential

*Proven experience of working with people with continence problems. *Evidence of personal and professional development.

Desirable

*Community experience. *Ability to demonstrate a comprehensive level of clinical knowledge and competence within the clinical sphere of practice

Additional Criteria Essential

Skills and Knowledge: *Good interpersonal and communication skills. *Good time management. *Eagerness to put forward new ideas. *Willingness to undertake group teaching *Evidence of good IT skills. *Good report writing skills.
