About this role

Job summary

We are looking for an enthusiastic, qualified nurse with relevant experience to join the Peritoneal Dialysis team, caring for patients receiving peritoneal dialysis at home.

You will be a proven team player with post-registration renal experience, including experience working at Band 5 level or equivalent. Previous experience caring for patients on peritoneal dialysis would be advantageous.

You will bring evidence of multi-disciplinary and multi-agency partnership working, along with the ability to both influence and embrace change. Well-developed communication skills are essential.

Main duties of the job

Key objectives include the management of patients receiving peritoneal dialysis, the initiation and training of new patients, and the provision of ongoing support and care to patients and their carers/families.

You will be expected to deliver high-quality care to this complex patient group, support patients in their homes and in outlying hospitals, and provide telephone triage, advice, and consultation.

You will play a key role in educating and supporting healthcare professionals, patients, and significant others throughout the patient journey, with the ultimate aim of enhancing quality of life.

A non-medical prescribing qualification is desirable but not essential, as training will be provided.

Due to the nature of the role, you will be required to travel to different geographical locations across the Trust footprint in line with service needs, and therefore must be able to travel independently.

Person Specification

Qualifications & Professional Registration Essential

Registered Nurse with current NMC registration (Part 1 Adult / General Level 1 or 2) First Degree in nursing or an equivalent portfolio of evidence Post registration experience within the speciality Evidence of involvement in clinical governance

Desirable

Have achieved or be working towards master's level qualification Post basic qualification in renal nursing, e.g., Renal Course Non medical prescribing

Knowledge, Training & Experience Essential

Previous experience of management of patient on peritoneal dialysis Excellent communication skills Renal experience Can demonstrate specialist expertise
