Job summary
The Remote/Hybrid IP Clinical Pharmacist will work as part of a Multidisciplinary Team and Pharmacy Team consisting of other Clinical Pharmacists, Pharmacy Technicians, and Pharmacy Administrators.
The post holder must be passionate about delivering high quality patient care, ensuring patient and drug safety, and in providing holistic patient centred care.
The Remote/Hybrid Clinical Pharmacist will conduct structured medication reviews, deprescribing where appropriate, covering care home patients, patients with polypharmacy, frailty and housebound. Supporting in PCN projects involving prescribing or deprescribing including Team up. Advising and working with the wider practice team with drug related queries, completing drug monitoring and drug safety processes.
The Remote/Hybrid Clinical Pharmacist will support the Pharmacy team, PCN Clinical Director and PCN Manager in ensuring targets around PCN DES, IIF and Practice QOF, drug safety, and drug monitoring, CQC and safety searches are met.
Main duties of the job
Conduct patient-facing medication reviews, primarily by telephone, with some face-to-face consultations based on PCN needs.Work as an Independent Prescriber within the multidisciplinary general practice and PCN team.Complete structured medication reviews for patients with long-term conditions, frailty, polypharmacy, care home residents, housebound patients, and people with learning disabilities or autism.Provide specialist medicines expertise to support public health, social care needs, and reduction of health inequalities.Support integration between general practice, community pharmacy, hospital pharmacy, and wider healthcare teams.Improve patient safety, quality of care, and access to healthcare services.Build and maintain strong working relationships with pharmacy professionals across PCNs and healthcare systems.Lead on shared care protocols, medicines-related clinical research, and liaison with specialist and community pharmacists.Participate in clinical audits and quality improvement activities to ensure safe, compliant, and evidence-based prescribing.Represent the PCN at ICB Medicines Management and relevant PCN/practice meetings as required.
