Job summary
An exciting opportunity has arisen for an individual to join our fast-growing friendly team as an Advanced Nurse Practitioner for 14 hours a week over 2 days, ideally on a Monday and Wednesday.
We are looking for a friendly, professional individual that strives to go the extra mile for their patients.
Main duties of the job
To carry out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.To assess, triage, diagnose and treat patients face to face accordingly, performing specialist assessment of their needs and make referrals as necessary.To request diagnostic investigations and interpret within agreed protocols e.g x-ray, blood tests and scans.To clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.As an independent prescriber, to review and prescribe medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols.
About us
South Hornchurch Medical Practice has approximately 6000 patients and we are located in a health centre with parking on site. You will be working alongside 1 Lead GP and 3 female locum doctors, an Advanced Clinical Prescriber, a Practice Nurse and HCA with additional PCN Staff including a Pharmacist, Physiotherapist, Dietician and a Social Prescriber. Person Specification
Experience Essential
Minimum of 5 years post registration experience and recent experience in a primary care/community setting. Evidence of appropriate continuing professional development activity to maintain up -to date knowledge and ongoing competence in all aspects of the ANP role, including long term condition management. Evidence of working autonomously and as part of a team. Proven ability to evaluate the safety and effectiveness of their own clinical practice.
Desirable
Knowledge of diabetes including initiation of insulin to patients.
Qualifications Essential
1. Degree in Nursing and UK NMC Pin Qualified 2. Master of Science in Advanced Nurse Practitioner
Desirable
RCN Minor injuries Certificate in Diabetes Care Managing patients with Chronic Diseases
