Job summary
An excellent opportunity has arisen for a Band 6 Health Care Professional within West Memory Assessment Team primarily based at Brookside , Ormskirk.
We are looking for an experienced professional with a background of working with people with dementia either in a hospital or community setting. You will have a passion for dementia and all aspects of assessment, diagnosis and supporting in delivering diagnosis followed by signposting to appropriate services.
You will be working with an experienced and well established team based in Ormskirk covering West Lancashire area. We also provide assessments in the community when needed.
You must be able to meet the travel requirements of the role
Main duties of the job
You will be joining a highly experienced and continuously developing team which is made up of Nurses, Support Workers, Occupational Therapists, Non- Medical Prescribers, Neuro-Psychologist, Consultant Psychiatrists who are supported by our admin and medical secretary teams
We work closely with 3rd sector support services including Alzheimer's Society and Age UK.
You will need to be able to demonstrate excellent organisational and time management skills, alongside clear communication and a desire to learn and develop the services we provide. We can provide excellent supervision, training and development.

Qualifications Essential
Recognised Core Mental Health Professional Qualification (e.g. RMN/Dip SW/OT)
Knowledge Essential
Experience in dementia and cognitive impairment Medications used for Dementia Cognitive problems and reversable causes.

Managing caseload /working under pressure Experience of effectively working with service users and carers Experience of delivering evidence based interventions in a time-limited framework
