About this role

Job summary

We are seeking a trained pharmacy technician to work under the supervision of the lead pharmacy technician to improve the quality of prescribing and patient outcomes through high quality medicines review . The post holder will work as part of a multi-disciplinary team, have excellent organisational and communication skills and also excellent IT skills.

Main duties of the job

The main duties of the role are-

Perform medicines improvement work to maximise safe, cost-effective best practice in prescribing under the supervision of the lead Pharmacy TechnicianAssist with medication safety monitoring systems (e.g high-risk drugs)Liaise with clinicians and administrative staff to resolve medicine- related queries.Support medicine reconciliation of patients whose care is transferred back into primary care in a timely and effective manner liaising with patients and other providers to ensure patients receive appropriate medication post dischargeCarry out medicines optimisation tasks including effective medicine administration, supporting medication reviews and medicines reconciliation of transfer of careCare Quality Commission-Work with the practice to ensure compliance with CQC standards where medicines are involvedImplement changes to medication that result from CAS/MHRA alerts, product withdrawal and other local/ national guidance

About us

We are a supportive practice in St Leonards on Sea in East Sussex providing high quality patient care to approximately 18,500 patients over 2 sites. We are a GP training practice. Our clinical team comprises of Salaried GP's, Advanced Nurse Practitioners, Advanced Clinical Practitioners (Paramedic and Pharmacist), Practice Nurses, Physiotherapists, Pharmacy Technician and HCAs. Our CQC rating is Good. We also have a wide team of support staff which include, Practice Manager, Medicine Management Assistant, Care Co-ordinators, medical secretaries, coders, scanners, prescription clerks, receptionists and general admin.

Job description Job responsibilities

Take responsibility for own development with relevant evidence based knowledge and competence in all aspects of the role to meet clinical governance guidelines for Continuing Practice Development Work to deliver the NHS contract requirements related to the Practice (including the terms of Quality and Outcomes Framework)Governance

Produce complete and accurate records of patient consultation in line with best practices, confidentiality, policies and procedures

Deliver care according to NHS guidance, NICE guidelines and evidence-based care Take part in maintaining quality governance systems and processes across the Practice and its activities. Awareness of statutory safeguarding, notification processes and local guidance for children/vulnerable patients, applying relevant policies and legislation to protect them Ensure compliance with policies, procedures and guidelines for self and others by taking action or alerting the senior management team if the Practice appears to contravene policy or if there are concerns over any aspect of patient care. Maintain confidentiality of information, acting within the Data Protection Act's terms and Caldicott's guidance on patient confidentiality. Maintain an awareness of the Freedom of Information Act.

Information relating to patients, carers, colleagues, healthcare workers and the practice business is restricted to authorised persons following the Practice policies and procedures regarding confidentiality and the protection of personal and sensitive data. Health and Safety

Person Specification

Qualifications Essential

Registration with the General Pharmaceutical Council (GPhC) BTEC/NVQ level 3 or equivalent in pharmaceutical science/pharmacy service Evidence of continued professional development

Experience Essential

Experience of working in a team

Desirable

Experience of working in primary care Experience of working in a GP practice
