We are seeking a dynamic and compassionate Senior Practitioner to join EIT in East Cornwall to work in this multi disciplined team. This is an exciting opportunity to work in a service that is committed to evidence-based practice and innovation.

As a Band 6 Senior Practitioner, you will:

Provide specialist assessment, care planning, and interventions for individuals aged 14-65 at risk of developing a first episode psychosis. Work collaboratively with service users, families, and carers to promote recovery, resilience and instil hope. Deliver psychosocial interventions, be a lead health care professional, and risk management in line with NICE guidelines. Contribute to service development and uphold the standards required for accreditation. Offer clinical leadership and supervision to junior staff and students.

Main duties of the job

To be responsible for the assessment of care needs and clinical management of a definedcaseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental HealthService.

To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system.

To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs.
