About this role

Job summary

As a registered, accountable and autonomous nurse act in the best interests of your patients at all times and take personal responsibility for your actions and decisions. To be responsible for patient assessment, planning of care, implementing and supervising safe and compassionate care, and evaluating the effectiveness of care. As an NMC registrant facilitate learning and development of others. You will demonstrate leadership qualities and be open, transparent and trustworthy. As a Trust employee act with integrity and uphold the values and beliefs of the Trust and the NMC Code of Conduct.

To act as lead and mentor for staff, supporting them in their development. To run clinics effectively ensuring a productive environment.

The role involves the management of quality, clinical care, and advocating for the patient experience and the effective management of resources.

Main duties of the job

To provide a high standard of individualised patient care reflecting individual competencies and education that involves relatives / carers as appropriate. To be responsible for the nursing care of a defined group of patients. To ensure that care is evidence based and patient centered. To assist in the management and organisation of the nursing service in the clinical area including the teaching and management of more junior staff or those in training.

The post holder will assist in delivering a service which meets target standards and promotes a creative and innovative approach to on-going service development, including the mentoring and management of junior staff, and driving forward Achieving Excellence within the department.

The post holder will be a resource nurse for the area and will act as a link in promoting an integrated specialist service. The post holder will develop elements of nurse led practice within LTC Hub.

Job description Job responsibilities

Professional function:

To adhere to the main standard of performance for patient care, within the concepts of the accepted nursing philosophy, The Guide and good customer care.

To carry out all relevant forms of nursing care without direct supervision.

To manage a group of patients ensuring effective communication with the nurse in charge.

To liaise with clinicians, other professionals and community staff as appropriate co-ordinating and maintaining care which will achieve the set standards.

To be aware of the ward budget in order to achieve a safe and cost-effective service within the constraints of the agreed budget.

To ensure care provided is evidence based and in line with the Trusts policies, procedures, standards and guidelines.

To act as a mentor and participate in teaching pre-registration students, Healthcare assistants and supporting National Vocational Qualifications as required ensuring they are given adequate supervision and support.

To be required to use any computer system purchased by the Trust providing appropriate training given.

To set an example of professional work and behaviour and may be required to take charge as required.

To participate in any other duties that you may be asked to undertake by senior nursing staff.

To provide cross-site cover across Hospitals including Royal Surrey, Milford Hospital and Haslemere Hospital as required, ensuring continuity of care and service delivery in line with departmental needs.

Promote a friendly and welcoming atmosphere for both team members and patients. Be courteous, helpful and supportive to patients at all time.

To liaise with clinicians, other professionals and community staff as appropriate co-ordinating and maintaining care.

To be involved with the systems and processes to ensure patients are appropriate referrals, onward care and review. You will be expected to record accurate data in accordance with the requirements for the service and your clinical procedures and work.

Personnel function:

To ensure that you are familiar with the Trusts policies and procedures and introduce staff to them as required.

To exercise leadership by example.

To continue to develop clinical and professional knowledge and be aware of your own training needs. Attend professional updates and mandatory training as dictated by the Trust and the NMC.

To work to the competencies/objectives expected of a Band 5 registered nurse.

To assist with appraisals of healthcare assistants.

In conjunction with the Charge Nurse / Senior Sister develop expertise in multiple nursing fields relevant to the LTC Hub and clinical setting, providing education and development for others i.e. Heart Failure

To support HCAs undertaking their relevant training and competencies acting as an assessor.

Supporting staff undertaking post registration education.

Act as a mentor to pre-registration students ensuring the promotion of a good learning environment.

Clinical Governance:

To participate in the clinical governance and quality activity in conjunction with the ward sister/charge nurse.

Contribute and lead where appropriate LTC Hub teaching sessions and be involved in producing a teaching resource file.

Be responsible as part of the team to manage, co-ordinate a high quality of patient information. Promote and participate in health education.

Person Specification

Qualifications Essential

Registered Nurse Part 1 or 12 Experience in the care and treatment of patients diagnosed with a long term health condition.

Knowledge and Experience Essential

Demonstrable clinical credibility Consolidation of Nurse Training Competencies in tasks such as IV additives/ECG recording and venepuncture or willingness to undertake
