Job summary

Are you an enthusiastic and motivated Occupational Therapist or Physiotherapist looking to develop your skills within a supportive and dynamic community therapy team? We have an exciting opportunity for a Band 6 Therapist to join our service, delivering high-quality, person-centred rehabilitation with a focus on supporting the transition from hospital to community.

As a Band 6 Therapist, you will work within the community team with a specific in-reach focus at University Hospitals Southampton. The role will involve assessing patients in hospital to support discharge planning, as well as following identified patients into the community to deliver ongoing rehabilitation. This includes providing intensive support to facilitate discharge home in line with the Home First model.

You will work alongside community therapy colleagues and the wider multidisciplinary team and to provide comprehensive rehabilitation and therapy interventions

You will manage a varied caseload, including complex cases, and contribute to service development and quality improvement initiatives.

You'll be joining a friendly, supportive team known for its collaborative spirit and commitment to patient-centred care. Our teams are embedded within their local communities, making a real difference to the lives of local people.

The role will be based within the local community, supporting patients in their own homes and community settings across the New Forest and Eastleigh areas.

Main duties of the job To provide specialist therapy skills within the Integrated Community Team working both in the community and 9 bedded inpatient unit.To assess and treat own caseload, which may include complex presentations and to maintain own records as an autonomous practitioner.To assist senior staff in the education and supervision of junior colleagues, assistants and students.To assist senior staff in daily management of the team and provision of therapy service, deputising for the team leader in their absence.To assist senior staff in development and maintenance of an efficient and effective therapy service.To contribute to the inter-disciplinary working within the Community therapy team and Locality by teaching, learning & practicing inter-disciplinary skills & techniques as defined by an agreed competency framework or training scheme.

Person Specification

Qualifications Essential

UK Health Professions Council registered Occupational Therapist or Physiotherapist Evidence of community experience and working as part of a multidisciplinary team.
