Job summary
To be responsible for implementing an effective medicine management system within the practice, including cost-effective prescribing, working as an independent prescriber, providing advice and support for both patients and colleagues. In addition, the post-holder will also support the multi-disciplinary team, in line with the strategic objectives of the practice
Main duties of the job
Provide clinical assessment and management of patients on anticoagulation therapy (including initiation, monitoring, and dose adjustment).
Prescribe and optimize anticoagulation treatment in line with local and national guidelines.
Conduct patient consultations, review INR results, and ensure timely follow-up.
Maintain accurate clinical records and contribute to audit and quality improvement initiatives.
Collaborate with GPs, nurses, and other healthcare professionals to ensure seamless patient care.
Support education and counselling for patients regarding anticoagulation therapy and adherence.

We are in the very fortunate position where all 4 of the Primary Care Networks (PCNs) in Redbridge are working together as part of the Redbridge-wide GP federation Healthbridge Direct.

We are now at the very start of an exciting journey of building a team of over 100 additional clinical roles to support and enable the delivery of services within each of our 4 PCNs. We are building teams within and across the PCNs to enable peer support and team working both within and across professions.

Person Specification
Qualifications Essential
Qualified Pharmacist with independent prescribing qualification. Current registration with the General Pharmaceutical Council (GPhC). Experience in anticoagulation management or relevant clinical areas. Strong communication and organisational skills. Ability to work autonomously and as part of a multidisciplinary team.
Desirable
Previous experience in primary care or community-based anticoagulation services. Familiarity with clinical systems (e.g., EMIS, INR Star).
