About this role

Job summary An exciting opportunity has arisen for a highly motivated and enthusiastic individual to join the Hull and East Riding Intermediate Care Team as a Therapy Associate Practitioner. You will work as part of a Multidisciplinary Team within a community rehabilitation unit, delivering safe, high-quality therapy interventions and rehabilitation programmes under the guidance of a therapist. You should be empathetic and have excellent communication and listening skills and the ability to support and motivate others to achieve their potential, building trusted relationships and helping people stay active, independent and engaged in their care.

Main duties of the job The organisation provides excellent CPD opportunities for staff, and you will receive training and support to develop your clinical knowledge and skills and work towards achieving Occupational Therapy and Physiotherapy competencies. You will manage a defined caseload and contribute to person-centred care plans. Record clear, accurate clinical information and share updates with the team. Providing support and guidance to new and less experienced staff. We are looking for a relevant Level 4 qualification, foundation degree, or equivalent experience in health or rehabilitation, working with people in a clinical or care setting. Ability to work independently while knowing when to seek support. Commitment to safe practice, learning andteam working. It is essential that you are a car driver, with a valid UK driving licence and have basic IT skills. Person Specification

Qualifications Essential

Relevant NVQ level 4 or Foundation Degree in Health and Social Care or demonstrable working experience in the field of rehabilitation

Knowledge Essential

Demonstrate awareness of healthy living practices Knowledge of the Health and Safety at Work Act Knowledge of preparing rehabilitation programmes for improving independence and fitness Knowledge of risks associated with performing exercise and when to escalate clinical concerns to the most appropriate clinician. Clear understanding of the rehabilitative process and role to play within this

Desirable

Knowledge of the Mental Capacity Act, Safeguarding Adults and Young People

Experience Essential

Experience of working within a clinical care setting Experience of engaging people in healthy living practices Experience of working with groups of people. Experience of working in a multi-disciplinary environment.

Evidence of working within rehabilitation clinical services Previous experience working with people with physical needs and adults with complex needs Experience of working with people with chronic/progressive disease Experience of liaising and working with multidisciplinary teams or care teams
