We are seeking a dedicated Practice Facilitator (Band 6) to support practice development across our Mental Health, Learning Disability and Neurodiversity (MHLDN) & Wellbeing Services at Livewell Southwest.

This is a rewarding opportunity to help strengthen clinical skills and confidence across teams, with a particular focus on embedding physical clinical skills into everyday mental health practice. You will support staff to deliver safe, effective, and holistic care and improve outcomes for service users. You will work closely with clinical and leadership teams to design, implement and deliver innovative training programmes, including classroom teaching, simulation, and real-world learning. Through your work, you will directly influence staff confidence, competence and quality of care.

This is a highly visible and impactful role, ideal for someone who is forward-thinking, collaborative, and committed to continuous improvement.

Working alongside experienced colleagues, you will contribute to the delivery of training and competency development, supporting staff to translate learning into practice.

Main duties of the job

Support the delivery of physical health training within mental health services, helping staff build confidence and competence

Assist in designing and delivering training sessions, including classroom teaching, simulation, and practical skills development

Promote and support the use of evidence-based practice, particularly in relation to physical health in mental health care

Work closely with the Practice Lead for Nursing and Deputy Head of Training to support workforce development initiatives

Act as a role model in clinical practice, maintaining strong professional standards and credibility

Build and maintain effective working relationships with clinical teams, managers, and partner organisations

Support staff with competency development and assessment, ensuring safe and consistent care delivery

Contribute to holistic assessments, ensuring physical health needs are recognised alongside mental health needs

Provide cover and support to the wider team as needed

You will work within your professional code of practice and Livewell Southwest policies at all times.

Competencies and Training:

To be involved in the planning, implementation, delivery and evaluation of clinical and essential training using relevant national and local guidelines and frameworks. Support in delivery of training programmes led by other clinical educators. Implement, participate, and facilitate in the development of Practice Supervision as peer groups for all staff groups across the Community Nursing teams. Facilitate the development of a peer learning groups for all staff groups across theCommunity Nursing teams. Develop opportunities to enhance development and sign off, of clinical practicecompetencies. Facilitate and support in the sign off, of clinical competencies, supporting evidencebased, quality leadership. Support with the delivery and implementation of essential clinical training for thesestaff groups. Provide expert clinical leadership and support to all staff groups across thecommunity Nursing teams. Student Nursing Associates and nurses acting as a role model in provision of cleardirection for nursing and practice development. The role includes a balance of clinical, educational and leadership responsibilitieswhich may vary according to need and to be agreed by the Practice lead for nursingto support service development.Responsibility for financial and/or physical resources

Ensure training equipment and consumables are maintained. Monitor stock levels and liaise with training team in regards ordering and restockingMaybe required to instigate ordering of new or updated equipment in line with current guidelines and best practice.Responsible for the safe use of equipment other than equipment which they personally use; And/or responsible for maintaining stock control and/or security of stock.Responsibility for Workforce and Operational Issues:

Works within the NMC Code of Practice.Ensure quality service delivery by developing and reviewing of workforce competencies in collaboration with professional training and development, practice lead for nursing and operational mangers.Support on strategies to support Professional training and development and Practice lead in the development of suitably skilled staff by using workforce planning developments to support the development of sill mix competency and development within the staff groups identified.To actively participate in supporting new ways of working and developing staff quality and competence within clinical practice.Develop opportunities to enhance development and sign off, of clinical practice competenciesRemain updated and competent and ensure that clinical practice is evidence based.Have an up-to-date personal development plan and professional portfolioParticipate in an annual appraisal of their work in line with the Knowledge and Skills Framework (KSF) where the job description will be reviewed, and objectives set.In line with the annual development plan the post holder will be expected to undertake any training or development required to fulfil their role.Responsibility for people who use our services

Provide nursing knowledge and experience to any member of the multi-disciplinary team in relation to patient care, ensuring a visible presence is maintained within clinical areas.Set, agree and monitor standards for clinical practice and patient care and ensure teams have the right skills to deliver a high-quality service.Ensure changes in clinical practice are reflected in training and competency assessment, alerting the team manager and practice lead for nursing where there is an unmet need.Work autonomously in the community setting, recognising the risks associated with lone working, e.g., aggressive behaviour, and work proactively to reduce those risks.Work as an autonomous practitioner, managing and prioritising own workload according to local guidelines and identified need. Working in a variety of community settings, exercising a high level of judgement, discretion and decision making in clinical care.To facilitate and support effective clinical assessment and transfer those skills to support quality of care and clinical competence for those identified staff in line with clinical assessments listed below:To Support and perform a comprehensive holistic assessment and physical examination for patients using clinical skills and knowledge to identify problems or symptoms. To be able to determine if symptoms warrant further investigation and/or treatment and initiate this as appropriate.To plan, implement and evaluate on an ongoing basis, care delivery using evidence based best practice.To be able to identify and resolve complex clinical issues.Support staff in providing expert clinical care and health promotion interventions that will help prevent unnecessary hospital admissions and periods of crisis. This will include the ordering and interpreting of diagnostic tests and act on results, referrals to relevant clinicians using clinical guidelines and policies to support clinical decision-making and the prescribing of medicines via independent and supplementary prescribing arrangements.To initiate and support medicines management incidents within our community teams linking with professional lead to identify themes and support with training.To support and facilitate effective Work with the multi-disciplinary team across health and social care to co-ordinate the development, implementation and evaluation of training.Responsibility for implementation of policy and/or service developments

The post holder will be required to use a computer, being responsible for timely recording

patient activities for IT data collection, dealing with e-mail queries, action development

plans.

Lead and participate in team activities so as to develop and consolidate a cohesive and

supportive team ensuring openness within the team.

You will be able to communicate complex, sensitive or contentious information. This may

be because agreement or cooperation is required or because there are barriers to

understanding. The Post Holder to be supplied with a mobile phone and for work purposes

Develop and maintain effective and appropriate communication networks with other

members of the Primary Health Care Team and other agencies.

Reporting and recording of all incidents and near misses as per policy.
