Job summary
Chapelfield Medical Centre is seeking an experienced, motivated, and patient-centred Practice Nurse to join our friendly multidisciplinary team.
This is an exciting opportunity for a skilled nurse with extensive experience in diabetes management and long-term condition care to play a key role in delivering high-quality primary healthcare services to our patient population.
The successful candidate will be NMC registered, hold a recognised Diabetes Diploma, and have proven experience managing patients with diabetes and other chronic diseases within a primary care setting.
Working closely with GPs, ANPs, healthcare assistants, pharmacists, and other members of the practice team, you will provide evidence-based nursing care and contribute to the ongoing development of clinical services.
Main duties of the job
Chronic Disease Management
Manage patients with long-term conditions including: Diabetes Asthma COPD Hypertension Cardiovascular disease Other chronic conditions as appropriate Undertake annual reviews and monitoring clinics. Develop and implement personalised care plans. Support patients in self-management and lifestyle modification. Provide education and advice to improve health outcomes. Clinical Nursing Duties
Cervical cytology Immunisations and vaccinations, including routine and travel vaccinations where appropriate. Wound care and dressings. Blood pressure monitoring and cardiovascular risk assessment. Phlebotomy. Contraception and sexual health advice (within competency and training). Health promotion and lifestyle advice. Screening and preventative healthcare interventions. Quality and Governance
Support the delivery of Quality and Outcomes Framework (QOF) targets. Contribute to Enhanced Services delivery. Maintain accurate and contemporaneous clinical records. Work within practice policies, protocols, and NMC guidelines. Participate in audit, quality improvement initiatives, and service development. Ensure patient confidentiality and information governance requirements are maintained. Participate in safeguarding processes and mandatory training requirements.

Person Specification
Qualifications Essential
Qualifications Registered Nurse with current NMC registration. Recognised Diabetes Diploma or equivalent accredited qualification. Evidence of continuing professional development. Experience Significant experience working as a Practice Nurse within General Practice or Primary Care. Extensive experience managing patients with diabetes. Experience managing other long-term conditions including asthma and COPD. Experience supporting achievement of QOF indicators. Experience working within multidisciplinary teams. Clinical Skills Competent in Chronic disease management. Diabetes reviews and management. Immunisations and vaccinations. Wound care and dressings. Phlebotomy. Blood pressure monitoring. Health promotion and lifestyle advice. Maintaining accurate clinical records. Working within evidence-based guidelines and protocols. Knowledge and Skills Excellent communication and interpersonal skills. Strong organisational and time-management abilities. Ability to work independently and as part of a team. Knowledge of current primary care and nursing practice standards. Good IT skills and experience using clinical systems.
Desirable
Independent Prescribing Qualification. Asthma and COPD diploma or accredited training. Travel Health qualification. Cervical cytology Experience using Systm One. Experience leading clinical audits or quality improvement projects.
