About this role

Job summary

We are delighted to offer the opportunity for a qualified General Practitioner to join our team at Asplands. We are looking for a GP to work with our 6 partners and a multi skilled clinical team. Our practice has recently been inspected by CQC and awarded Good status.

The successful candidate will provide both routine care and long-term condition reviews. The role requires working autonomously, providing assessments, diagnosis, treatment plans and pro-active care.

We are looking for a qualified GP for between 2 and 4 sessions per week and are happy to discuss flexibility around the number of days and timings of those sessions.

Main duties of the job

The GP will undertake a variety of duties governed by the GMS contract and objectives of Enhanced services, including:

In person consultations Some telephone consultations Visiting patients in their homes or care home Checking and signing repeat prescriptions Dealing with queries, paperwork and correspondence as required Clinical administration including processing of results and coordinating referrals for specialist review

Asplands Medical Centre offers an exceptional opportunity to join a welcoming and friendly team. The practice is led by 6 GP Partners and support is given by 1 salaried GP and an Associate GP. In the heart of our practice is an extremely supportive nursing team with specialities that include Respiratory, Diabetes and HRT.

Asplands is a busy semi rural practice with almost 13,000 patients. We operate over two sites and have a Dispensary at both sites. We work closely with colleagues from Ascent PCN including Social Prescribers, an Occupational Therapist, a Physiotherapist and a Mental Health Nurse.

We have a strong ethos of patient centred care and operate a usual GP approach. This helps to support a pro-active approach to patient care.

Education is valued at Asplands Medical Centre and as such we train GP Registrars, Medical Students, Pharmacy Students, Nurses and Associate Nurses. We also actively encourage staff to develop their own skills and qualifications. Asplands also takes part in research and we are looking to develop more projects in the future that create opportunites for both our team and our pateints.

Job description Job responsibilities

As a Salaried GP, you will provide autonomous care for our patients and encourage pro-active management. You will demonstrate safe clinical decision making and provide expert care by undertaking history taking, clinical assessment, diagnosis, treatment and evaluation of care, including appropriate follow-ups.

Clinical

Provide clinical care to our patient; assessment and management of patients mainly via face to face appointments. Use skills and expert knowledge to assess the physical and psycho social needs of patients with undifferentiated primary care conditions including minor illness and instigating therapeutic treatments based on best available evidence to improve health outcomes. Use advanced skills and expert knowledge to make comprehensive and focussed assessments, independently and proactively managing care of the patients. Promote patient independence and self-care where appropriate Support and work cohesively in a team with other GPs/ANPs and ACPs including providing oversight and mentorship for more junior colleagues. Review pathology results and other provider communications to ensure appropriate treatment and care Whilst considering the patient's needs, deliver on targets for the Quality and Outcomes Framework and NHS England/ICB/PCN ContractsRegulatory

Respond to complaints and clinical incidents/significant events as appropriate to ensure robust systems are in place for improving practice through joint learning. Maintain accurate and timely clinical records in line with professional and legal standards. Undertake at least one clinical audit a year. Evaluate data and present findings to the Practice Clinical team. Implement changes where appropriate. Adhere to the employing practice Policies and Procedures including Infection Control, Health and Safety, and safeguarding. Ensure all mandatory training is completed in a timely manner. Maintain own personal/professional development plan and portfolio thereby meeting the requirements for revalidation through appraisal of own performance. Provide Mentorship and training of new recruits or students as and when required. Participate in the Practice Performance Review process.
