Job summary

This is a rare and exciting opportunity to be part of the Herefordshire and Worcestershire Health and Care Trust Advanced Clinical Practitioner (ACP) team working within The Princess of Wales Community Hospital. We are seeking an experienced and passionate ACP for Primrose Unit, specialist inpatient palliative care unit to work alongside the nursing, medical and multidisciplinary team at an advanced practice level applying their extended knowledge and skills in the principles and practice of palliative care to support patients with the most complex needs.

A Band 8a position will be offered to candidates who have completed an MSc in Advanced Practice and can demonstrate experience working at an advanced level within a relevant clinical environment. This is a part time position of 24 hours per week over 3 days.

Main duties of the job

The successful applicant will work autonomously to admit and assess individual patient needs, determine a differential diagnosis, request investigations, and initiate appropriate holistic, research-based treatments, within a patient centred clinical management plan, to include independent prescribing. They will be expected to exercise advanced clinical expertise, and high levels of decision making in clinical care, whilst working closely with Medical, Nursing and Therapy teams to ensure patients receive appropriate holistic care, and co-ordinate a safe discharge with minimal avoidable delays. Additional responsibilities will include monitoring and leading on improvements to standards of care through, supervision of practice, clinical audit, evidence-based practice, facilitation of learning through teaching and supporting professional colleagues, and demonstrate enhanced professional leadership skills.

The successful candidate will be expected to provide high quality specialist palliative care comprising expert advice and holistic assessment whilst managing patients with difficult life limiting conditions. This post requires a dedicated individual comfortable practicing autonomously with advanced practice in clinical decision making and the ability to formulate complex management plans to meet the needs of the patient and those close to them.

The post holder will demonstrate advanced communication skills to proactively manage and anticipate future health and social care needs, ensuring the fundamentals of future planning are embedded. In conjunction with the nursing and medical team, the post holder will be expected to formulate clinical guidance for all aspects of clinical practice within the clinical speciality. Key to this role is provision of high level visible clinical leadership which combines expert practice and a strong professional commitment to improve patient outcomes and champion innovative practice within palliative and end of life care.

The successful candidate should be able to combine expert practice with education and research, involving not only the specialist nursing team, but colleagues across the community hospitals linking with trust wide projects and strategic aims.

Experience Essential

Experience in palliative care in both acute and community setting

Qualifications Essential

Registered Nurse Level 1 / Allied Health Professional / Paramedic / Pharmacist MSc Advanced Clinical Practice Non-Medical Prescribing (V300) / ability to work within PGDs, within scope of professional practice. Advanced Assessment Skills Module Evidence of a portfolio of knowledge, skills and professional development I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
