Job summary
We are seeking an enthusiastic and highly motivated Senior Staff Nurse to join our friendly and innovative Community Intervention Service. This is an exciting opportunity for an experienced community nurse to take a leading role in delivering high-quality, patient-centred care across Tamworth and the wider locality.
You will work closely with the Lead Practitioner, and Operational Lead to support service delivery, staff development, and the continued evolution of community nursing in response to local population needs.

Main duties of the job
Provide clinical leadership to the Nursing Team, ensuring safe, effective, and high-quality care deliveryManage a clinical caseload across a wide geographical area, delivering care within patients' homes.Undertake comprehensive clinical assessments, planning, implementing, and evaluating care pathwaysDeliver a wide range of clinical interventions, including Administration of medications via injections and intravenous routesECG monitoringWound assessment and managementCatheter care and line care.Support and supervise staff to enable the consistent delivery of high-quality care.Work collaboratively with Lead Practitioner and Operational Lead to drive service development and performanceEngage with complex patients to develop mutually agreed treatment plans and deliver evidence-based careLiaise effectively with multidisciplinary teams to ensure seamless care pathwaysFacilitate early hospital discharge and contribute to the prevention of hospital admissions. Act as an effective leader, while role modelling Trust valuesSupport staff development through mentorship, supervision, and sharing clinical expertiseContribute to quality assurance processes, including audit, incident reporting, and patient feedback, working with the leadership team to evaluate outcomes.Take responsibility for the management of staff and service delivery in the absence of the Clinical Lead/Operational LeadDemonstrate the ability to make sound, independent clinical decisions
