About this role

Job summary

We are looking to recruit an enthusiastic and highly motivated Advanced Nurse Practitioner to join our friendly and progressive practice team. We are a four-site practice providing high-quality primary care services to a patient population of approximately 17,000 across the Longton and Meir areas of Stoke-on-Trent.

The successful candidate will work as an autonomous practitioner within the multidisciplinary team, delivering high standards of clinical care. You will undertake the assessment, diagnosis, and management of patients presenting with a wide range of minor illnesses and injuries, as well as acute and long-term conditions.

You will be expected to practice safely and effectively within your scope of practice, demonstrating clinical decision-making and leadership skills. The role includes initiating appropriate investigations, treatment plans, and referrals in line with local and national guidelines.

Main duties of the job

The post holder will be an experienced and competent Advanced Nurse Practitioner with a strong background in general practice, demonstrating expert clinical knowledge and advanced practice skills.

You will be required to work autonomously within your scope of practice, exercising a high level of clinical judgement in the assessment, diagnosis, and management of patients. You will provide safe, effective, and evidence-based care, managing a broad spectrum of undifferentiated and complex presentations.

The role requires independent prescribing, with responsibility for initiating, reviewing, and monitoring appropriate treatment plans. You will be expected to recognise the limits of your practice, seeking advice or referring patients to other healthcare professionals and specialist services when appropriate.

In addition, you will ensure that patients are fully informed about proposed treatments and procedures, obtaining valid consent in line with professional and legal standards. Accurate, timely, and comprehensive documentation of all consultations and clinical decisions must be maintained within the EMIS Web clinical system, in accordance with local and national policies.

Key ResponsibilitiesThe Advanced Nurse Practitioner will:

Work autonomously within their scope of practice and be accountable for their professional actions, demonstrating a high level of clinical judgement and decision-making.Deliver advanced clinical nursing care using expert knowledge and skills, providing holistic, patient-centred care to the registered practice population.Assess and manage patients presenting with a wide range of acute, non-acute, and long-term conditions, including taking comprehensive histories, performing clinical examinations, developing differential diagnoses, and implementing appropriate management and treatment plans, ensuring suitable follow-up arrangements.Initiate referrals to other healthcare professionals and services within primary and secondary care as appropriate, and arrange hospital admissions when clinically indicated.Undertake telephone triage and provide clinical advice remotely, ensuring safe and effective patient management.Prescribe medications in accordance with current legislation, local policies, and clinical guidelines.Maintain accurate, contemporaneous, and comprehensive clinical records within the EMIS Web system, in line with professional and organisational standards.Carry out medication reviews and contribute to effective medicines management.Request, interpret, and act upon diagnostic tests and investigations in line with patient needs and best practice.Participate in clinical audit, quality improvement initiatives, and service development.Actively contribute to multidisciplinary team working, including participation in clinical meetings and practice discussions.

Person Specification

Qualifications Essential

Registered Nurse with Nursing and Midwifery Council Masters degree required for qualification post December 2020 (See RCN Credentialing for Advanced Level of Nursing Practice) Post graduate diploma or degree for Advanced Practice Qualification up to December 2020 Qualified Independent Nurse Prescriber on the NMC register Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet Meets the standards for registered ANP working at advanced level

Desirable

Qualified triage nurse Minor illness qualification

Experience Essential

Experience of working in Primary Care. Development of areas of clinical expertise. Nurse-led management of minor illnesses and minor ailments. Management of patients with long term conditions. Nurse-led triage. Developing protocols and clinical guidelines. Advanced clinical practice skills

Desirable

Minimum 2 years recent experience in a primary care setting
