22.5 hours per week.

As a Band 5 Community Nurse, you will deliver high-quality, patient-centred nursing care to individuals in their own homes or community settings across the South Hams area. You will work as part of a multidisciplinary team (MDT) supporting people to maintain independence, manage long-term conditions, and avoid hospital admissions wherever possible. This position is pivotal in the shift from hospital based care to preventative neighbourhood care and response.

Main duties of the job

As a Band 5 Community Nurse, you will deliver holistic, patient-centred care to individuals in their own homes and community settings, carrying out comprehensive assessments and planning, implementing, and evaluating care to meet a range of physical and health needs. You will provide clinical interventions such as wound care, medication administration, catheter care, and support for long-term conditions, while promoting independence, health education, and self-management. The role requires strong teamwork and collaboration, working closely with multidisciplinary colleagues including GPs, senior nurses, therapists, and social care professionals to ensure coordinated, high-quality care and safe hospital discharge. You will maintain accurate clinical records, adhere to the NMC Code and safeguarding policies, and contribute to service improvement, clinical supervision, and continued professional development while often working autonomously across the community.

Digital skills for a Band 5 Community Nurse include the ability to confidently use electronic patient record systems to accurately document care, update patient information, and access relevant clinical data in a timely manner. The role requires good general computer literacy, including the use of email, digital communication tools, and basic Microsoft Office applications for record-keeping, reporting, and information sharing. Nurses should be able to input and retrieve data effectively, maintain confidentiality, and follow information governance standards. Additionally, the ability to interpret basic audit data and service performance information is important to support quality improvement and evidence-based practice. Competency in using digital devices in the community, such as laptops, tablets, and mobile phones, is essential, along with the adaptability to learn new systems as healthcare technology evolves.

Experience Essential

The essential criteria for the Band 5 Community Nurse role include holding a first-level nursing qualification (degree or diploma) with current NMC registration, along with a commitment to ongoing professional development and willingness to undertake further training in long-term condition management. The post requires the ability to work both independently and as part of a team, with strong communication skills to engage compassionately with patients, carers, and colleagues. Applicants must demonstrate up-to-date clinical knowledge, an understanding of community nursing practice and clinical supervision, and experience across a range of healthcare settings. Key skills include problem-solving, decision-making, time management, and the ability to assess patients, prioritise care, and make appropriate referrals using evidence-based practice. The role also demands excellent interpersonal and organisational skills, the ability to work under pressure, adapt to changing environments, maintain accurate records, and work within organisational policies. Additionally, candidates must be able to work flexibly across shift patterns, uphold high standards of care, and safely perform essential clinical procedures such as venepuncture, wound care, medication administration, and complex nursing interventions. Ability to frequently travel independently and in a timely manner between Livewell Southwest sites and community locations including areas not served by public transport.
