About this role

Job summary

This is an exciting opportunity for a motivated and patient-focused pharmacist to join a growing PCN where you can make a real difference every day. Liberty PCN is based at the modern St Georges Health and Wellbeing Hub, you will be part of an ambitious team committed to delivering high-quality, person-centred care to our local community.

In this rewarding patient-facing role, you will work alongside GPs and a wider multi-disciplinary team, using your expert knowledge of medicines to assess, treat and support patients across a broad range of clinical needs. You will join a well-established pharmacy team, being aligned to one of our practices, as per our clinical pharmacy team and senior clinical pharmacist support, giving you excellent peer support, collaboration and shared learning.

You will play a key role in improving care for patients with long-term conditions, leading clinical medication reviews and proactively supporting people with complex polypharmacy. This is a role where your expertise will have visible impact, particularly for older people, people in care homes, people living with multiple long-term conditions, and those with learning disabilities or autism.

Main duties of the job

To be a prescriber, or completing training to become prescribers and work with and alongside the general practice team

To undertake the care management of patients with chronic diseases and to undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities (frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP)

Provide expertise in the use of medicines and person-centred medicines optimisation across the network, including taking a central role in the clinical aspects of shared care protocols and liaison with specialist pharmacists to reduce inappropriate use of high-risk medicines

Support the integration of GP with wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, reduce waste, promote self-care, provide better access to healthcare, and manage GP workload.

Assisting with QOF and incentive scheme audits, PINCER safety indicators

Attending Medicines Management Prescribing forums quarterly and assisting practices in achieving Prescribing incentive Scheme targets.

CQC preparedness

Care home support

Discharge summary reconciliation

Pharmacy Medication reviews.Minor ailment clinic, Respiratory, Diabetes, Cardiovascular Colostomy products etc

Havering Liberty Primary Care Network is a collaborative network of five GP practices serving the communities of Upminster and Hornchurch. Key Duties and Responsibilities include:

The PCN Clinical Pharmacist responsibilities include but are not limited to the following:

To be a prescriber, or completing training to become prescribers and work with and alongside the general practice team

Provide expertise in the use of medicines and person-centred medicines optimisation across the network, including taking a central role in the clinical aspects of shared care protocols and liaison with specialist pharmacists to reduce inappropriate use of high-risk medicines

Support the integration of general practice with wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, reduce waste, and promote self-care, provide better access to healthcare, and manage general practice workload.

Managing Safety and CAS alerts

Assisting with QOF and incentive scheme audits, PINCER safety indicators

Attending Medicines Management Prescribing forums quarterly and assisting practices in achieving Prescribing incentive Scheme targets.

CQC preparedness e.g. ensuring all prescribing protocols are up to date

Care home support e.g. annual meds review of patient

Telephone advice to patients for queries on medication and minor illness, self-care, sign posting to other services as needed.

Review of repeat prescription policies and support staff in implementation.

PCN Pharmacy Team, including Clinical Pharmacists and Pharmacy Technicians working within the PCN/across practices

GPs, nurses and other practice staff

Practice based GP prescribing Lead(s).

Community nurses and other allied health professionals including dietetics.

Community pharmacists and support staff

Hospital staff with responsibilities for prescribing and medicines optimisation

Care Home Staff

The voluntary sector. Receiving referrals from Locality or Practice based Social Prescribers.

The Multi-Disciplinary Team

Education, learning and development

It is the responsibility of the employee to comply with all organisational and statutory requirements (eg. health and safety, infection control, equality and diversity, confidentiality, safeguarding adults and children, information governance).

Engages in annual appraisal, developing objectives to inform a Personal Development Plan, which may include 360-degree appraisal and use of patient feedback.

Participate in teaching and training of medical, nursing, and all other practice staff.

Supports the practice staff and responds to requests for advice and assistance.

Complete all mandatory and statutory training required by the role.

Undertakes additional training where necessary to provide enhanced services and participate in training programmes implemented by the PCN/practices as part of this employment.

Under supervision and support of GPs in the practice, the post-holder will strive to maintain quality within the practices, and will:

Participate in clinical governance activity and contribute to the improvement in quality of health outcomes through audit, risk management and Quality Improvement

Alerts other team members to concerns about risk, quality, and safety

Participates in investigation of incidents and events as required

Identifies, applies, and disseminates research findings relating to own practice

Collects data for audit purposes and uses clinical audit to monitor quality in the service

Contributes to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance

Works effectively with individuals in other agencies to meet patients needs

Effectively manages own time, workload, and resources

Meets timescales/deadlines for audits and written returns to ensure that the Practice meets quality standards and receives the designated funding.

For clinical staff with direct patient contact, this will include compliance with clinical procedures and protocols, including uniform and dress code, the use of personal protective equipment policy, safe procedures for using aseptic techniques, and safe disposal of sharps
