Job summary Clinical Nurse Specialist - Inclusion Health Outreach Band 6 We are seeking an experienced registered nurse to join the Find&Treat Inclusion Health outreach service, delivering HIV-focused clinical care within a generalist inclusion health outreach model. This role will support people living with HIV who are out of care or at risk of disengagement, alongside delivering BBV testing, TB and STI screening, and indicated vaccinations among people experiencing homelessness, substance use, migration-related vulnerability and prison experience

Main duties of the job As part of a multidisciplinary team, the Clinical Nurse Specialist will: o Support re-engagement of people living with HIV into care, including clinical assessment and facilitation of linkage to HIV services o Undertake BBV testing and HIV-related bloods (e.g. viral load, CD4) where appropriate o Deliver TB and STI screening in community settings o Deliver indicated vaccinations in outreach settings as part of holistic care o Identify and respond to wider health and social needs impacting engagement (e.g. housing, substance use, mental health) o Work collaboratively with peer practitioners to improve engagement, trust, and uptake of services o Support individuals into wider healthcare pathways including HIV, TB, hepatology and primary care services o Contribute to audit, performance monitoring and service improvement This is a generalist inclusion health nursing role with a strong HIV focus, requiring flexibility across blood-borne viruses, TB, and sexual health pathways. The role involves travel across London and working in varied community environments. About Find&Treat Find&Treat is a specialist NHS outreach service working with over 200 NHS and third-sector partners to tackle TB, HIV and blood-borne viruses among people experiencing homelessness and social exclusion. We take healthcare into the community, identifying infection early, supporting engagement with care, and addressing the wider health and social needs that underpin long-term outcomes.

Person Specification

Qualifications/Experience Essential

oRegistered Nurse (Adult) with current NMC registration. oEvidence of post-registration experience in community, infectious diseases, Inclusion Health, or related clinical settings. oCompetence to practise under Patient Group Directions (PGDs). oKnowledge of tuberculosis (TB), blood borne viruses (BBVs), and infectious disease management. oKnowledge of seasonal influenza vaccination protocols and immunisation standards oUnderstanding of safeguarding legislation and escalation procedures. oKnowledge of clinical governance, risk management, and quality improvement processes.

Desirable

oExperience working within Inclusion Health or outreach settings. oEvidence of leadership or supervision training.

Experience Essential

oExperience delivering autonomous clinical assessment and care planning. oExperience working with socially excluded or marginalised populations oExperience working within a multidisciplinary team. oExperience participating in audit or quality improvement activity. oExperience managing competing priorities within outreach or community settings.

Desirable
