About this role

Job summary

We are looking for a RSCN / RN Child nurse to join our Childrens Community Nursing Team delivering care to children within a range of community settings, including family homes, schools, nurseries and clinic. The post will involve providing nursing care for children and young people with complex health needs, acute and long-term conditions. You should be able to demonstrate relevant and current post registration experience of care delivery to children / families. The ideal candidate will possess good communication and organisational skills and have the ability to work both independently and as part of a team.

Main duties of the job

As a Childrens Community Nurse, you will provide specialist nursing care to children and young people with acute, complex, chronic, life-limiting, or additional health needs in a range of community settings, including the childs home, school, and community clinic. The role involves assessing health needs, planning and delivering evidence-based care, supporting hospital discharge and working in partnership with children, families, schools, and the wider multidisciplinary team. The Childrens Community Nurse provides education and training to parents, carers, and school staff. ensure that care is child-centred, safe, and responsive to the childs developmental, emotional, and social needs.

Job description Job responsibilities

Job Purpose:

To work within the Childrens Community Nursing Service, delivering high quality, evidence-based nursing care for children and young people with short- and long-term conditions.

To identify the health needs of children to be nursed in the community and work in liaison with multi-agency partners to promote collaboration and effective communication.

To work across the Childrens Community Nursing Service developing skills in complex, acute and long-term conditions care.

Duties and Responsibilities:

Post holder to work flexibly across the Childrens Community Nursing service, delivering high quality, evidence-based nursing care for children and young people with short- and long-term conditions including supporting children with life limiting and life-threatening conditions, preventing hospital admission and facilitating early discharge.

To identify the health needs of children to be nursed in the community and work in liaison with multi-agency partners to promote collaboration and effective communication.

To facilitate early discharge from the hospital/A & E by providing care at home as an alternative to hospital admission. To act as a specialist resource developing strong links with General Practitioners, Health Visitors and School Nurses.

To assist in the ongoing development and management of the Childrens Community Nursing Service.

To accept responsibility for own professional practice as a reflective, accountable clinical practitioner.

Clinical Management and Care Delivery

Receive referrals into the service and ensure robust assessment, planning, implementation, and evaluation of programmes of care that are family centred and foster the principles of clinical excellence.

Deliver high quality nursing care, health promotion and education that contributes to well-being and illness prevention, by ensuring both clinical practice and all verbal and written information is evidence based.

Ensure care at the point of delivery is in accordance with the Trust agreed clinical standards, policies and guidelines and that care is given in a competent manner that recognises own limitations and seeks appropriate supervision as necessary.

Possess up to date clinical skills to closely monitor the childs progress and improvement, and to detect deterioration promptly following the agreed care pathway to medical reassessment.

Provide accurate updates to senior staff of the childs physical and emotional wellbeing and on the progress of their condition / diagnosis. Report promptly any deterioration in their condition.

Undertake clinical tasks, which require advanced skills, e.g. administration of IV antibiotics, respecting the rights of children and young people and ensure appropriate consent is obtained prior to carrying out any clinical procedures.

Actively participate in the supervision and support of junior staff, delegating and prioritising duties, tasks and workload.

Contribute to efficient budget management, by optimising use of resources, reporting any anticipated budgetary needs or pressures.

Participate in the discharge process, both to and from the service, ensuring appropriate referral onto other healthcare staff or agencies if care dictates.

Be able to provide accurate up to date activity data related to service provision.

The post holder shall ensure that all children with additional needs are provided with a co-ordinated multi-agency response to meeting their needs.

To work flexibly to cover the needs and specification of the service.

Communication and Organisational Awareness

Promote good communication links between primary, secondary and tertiary services, endeavouring to foster the principles of seamless care.

Attend and participate in multi-disciplinary meetings, safeguarding procedures and case conferences as required.

Be aware and participate in risk assessment for the benefit of self, colleagues and clients in line with current trust policies, with particular emphasis on the Lone Worker Policy.

Ensure that verbal communications and written documentation are maintained and are clear, relevant and concise, in accordance with Nursing and Midwifery Councils (NMC) Standards of Records and Record Keeping and current Trust policy.

Maintain confidentiality of information and ensure safe custody of all records, in accordance with Trust policies and NMC guidelines.

Actively promote the services philosophy and values to support the child and family, whilst always acting as an advocate for the child and family.

Be responsible for reporting any accidents, incidents or complaints as per Trust policy.

Education and Training

Provide advice and training on the management and treatment of paediatric conditions. Support children and families according to their level of knowledge, ability and willingness to participate in care delivery.

Participate in the development of staff within the service, promoting professional development within the service in order to develop skills and competencies, in accordance with the Knowledge and Skills Framework (KSF).

Assist and support training and assessment of junior staff, childrens support workers, student nurses and education staff, ensuring their learning outcomes link theory to clinical practice. Participate in the induction programmes of newly appointed staff; attend Trust mandatory training sessions as required.

Participate in the delivery of teaching sessions as part of the day-to-day delivery of the service. Within the service, promote teamwork, share new skills, knowledge and expertise, which is proven best practice and evidence based.

Experience Essential

Post registration experience Able to effectively supervise/support fellow team members and student nurses.

Desirable

Use skills to teach/develop fellow team members, student nurses, parent/carers and education staff Able to undertake Practice Assessor / Supervisor role.

Qualifications Essential

1st level Registration RSCN/RN Child To possess or be willing to complete the Paediatric Clinical Examination and Minor illness module.

Desirable

Post Reg education. To possess or willing to work towards BSc in Community Childrens Nursing.

Skills and Knowledge Essential

Relevant experience working in a healthcare setting with children and their families, with complex health needs / acute health needs.

Desirable

IT skills. Teaching, audit and evaluation skills. Caring for children within a community setting. Completion of an Intravenous therapy course
