The post will form part of the Acute Care at Home Operational team, which is currently based at Smithfield One building, Hanley. Acute Care at Home is a countywide admission avoidance hospital service that provides a Community Rapid Intervention Service (CRIS) - 2hr urgent response, Virtual Wards and an Acute Visiting Service. The post is for a countywide role that is a secondment that can support the Team for 6months whilst we assess the value of having the role permanently. It gives staff the opportunity who have or are interested in a different part of the Trust that will give them additional skills, knowledge, awareness and experience
Main duties of the job
The role is pivotal in supporting the Operational Managers and Matrons in ensuring the right resources are available across the Teams, supporting in operational planning, service development opportunities, achieving performance targets and reporting and demonstrating continual improvement of high quality patient care across the services.
It would be providing countywide support so may need to work and attend meetings in other bases used by the Service

Communication and Relationship skills
2years experience in an Acute Trust or similar NHS setting
Project management and operational support across the key services
monitoring of performance and presenting to the senior management team
analysis and interpretation on a range of information in order to understand the services and financial and clinical performance and activity
ensuring that systems are developed to monitor Care Group and Service Lines of performance in relation to national and local standards such as the management of referrals, response levels, patient standards, clinical governance and the NHS long term plan
Incidental contact with patients
to support the service to deliver value for money and the planning of associated cost improvements and efficiencies across services

Educated to degree level or relevant experience
Desirable
Diploma/Degree in Management or health related subject
Knowledge, Skills and Experience Essential
2yrs experience of working in an Acute Trust/NHS organisation in a management role Operational management skills and experience Extensive project management experience Diplomacy and ability to deal with difficult situations Leadership and team building skills Ability to work in high pressured environments and work to deadlines Excellent communication skills oral and written Ability to develop and deliver action plans in relation to NHS agendas Knowledge of business planning processes Able to positively influence to ensure effective and efficient delivery of the service
