About this role

Job summary

The post holder will work as part of a multi-disciplinary team in a patient facing role. They will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, especially for older people, people in residential care homes and those with multiple co-morbidities.

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice(s).

The post holder will be involved with medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

About us

Newhall Surgery has a list size of approximately 11,200 patients. We have an active Patient Participation Group and endeavour to liaise well within the local community. We are active on Facebook and have good patient reviews.

Our clinical staff consists of four Partners, 6 GPs, 2 GP Registrars, 3 Trainee Doctors, an Advanced Clinical Practitioner, Advanced Nurse Practitioner / Nurse Manager, a Nursing Team of 4, 2 Clinical Pharmacists, 2 Care Coordinators and a GP Assistant.

We also have committed and professional management, administrative, secretarial and care navigation teams.

We are a friendly Practice who believe that communication, team working, well-being and learning are vital to the success of our organisation.

We have 'get togethers' every year to help support our internal communication processes and the feeling of 'family' within the organisation.

Person Specification

Qualifications Essential

Masters degree in pharmacy (MPharm) (or equivalent)

Desirable

Membership of the Royal Pharmaceutical Society A member of working towards Faculty membership of the Royal Pharmaceutical Society

Experience Essential

In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare. An appreciation of the nature of GPs and general practices An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing Excellent interpersonal, influencing and negotiating skills Excellent written and verbal communication skills Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients) Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions Good IT skills Able to obtain and analyse complex technical information Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate Able to work under pressure and meet deadlines Produce timely and informative reports Gain acceptance from recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers Works effectively independently and as a team member Demonstrates accountability for delivering professional expertise and direct service provision
