Job summary

This is an exciting opportunity for existing clinicians or newly qualified professionals to join the Dementia Intensive Support Team based in Bury St Edmunds. This is a full time (37.5 Hours) post.

You will be working in a highly skilled multi-disciplinary team with a strong team ethos of supporting each other. You will be supported to develop a strong understanding of your role as a professional in the team, becoming skilled at delivering evidence-based interventions for our service users. Working in partnership with other agencies will be a big part of the job so good communication skills will be vital.

Main duties of the job

Day to day the role will involve core duties some of which are listed below:

You will enjoy plenty of opportunities for joint working, training, and a sound supervision structure.

You can expect a manageable case load and a supportive management team to ensure you're able to work independently but support is never far away if needed.

The ideal candidate should have experience in working in a mental health environment and ideally experience in working in community teams with people with cognitive impairment.

Working in a busy and dynamic Dementia Intensive Support Team means every day will be different:

Managing a caseload via face to face, online and telephone contacts Undertaking initial assessments and triage on behalf of the service Undertaking face to face visits in the community setting including home and care home visits Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders Attending regular MDT meetings and liaising with colleagues from other services (both internally and externally such as Social Care, Crisis Resolution and Home Treatment Team, Adult Community Mental Health Teams, Acute Hospitals A&E, and third sector organisations) **Valid UK Driving licence is required for this post**

Qualifications Essential

Degree or Diploma in Occupational Therapy Or, Nursing degree or equivalent RN (MH) Professional Registration
