Job summary

To cover maternity leave we have the opportunity for a suitably qualified RN to join our Frailty Teams on a band 6 Secondment role. As part of this exciting, varied role you will be working with the Acute Frailty Team, Frailty Assessment Unit and Hospital at Home services.

Main duties:

Conduct initial triage and assess patients who present to the Frailty Assessment Unit, Hospital at Home and Acute Frailty Team.

Perform a comprehensive assessment of the patient's condition, including physical exams, history taking, and diagnostic tests.

Prioritise and provide appropriate care, ensuring patients receive timely interventions.

Administer medications, treatments, and IV therapy as needed.

Rapid Diagnosis and Treatment:

Collaborate with doctors and other healthcare professionals to quickly diagnose and treat patients.

Perform or assist in minor procedures, such as wound care, suturing, and administering injections.

Ensure patients are stabilised and treated efficiently.

Main duties of the job

The successful candidate will provide leadership, clinical expertise and organisation working between the 3 Frailty Teams.

They will continuously develop and enhance knowledge and skills that will directly influence consistent standards of clinical practice and ensure the highest possible standards of evidence-based care are achieved in order to promote patient focused care.

They will promote patient centred high quality evidence-based care to patients, ensuring the patient is central to all aspects of treatment and care.

Communicate effectively with all the multi- disciplinary teams both internal and external to the organisation.

Provide clinical leadership to a group of staff within the ward and throughout the care group.

Patient Assessment and Care:

Patient Care Management:

Oversee the delivery of patient care within the clinical area.

Ensure that care is delivered in accordance with clinical standards and patient needs. Supervise and

assist in the administration of medications and treatments.

Maintain detailed patient care records and update care plans regularly.

Leadership and Team Management:

Lead, mentor, and supervise a team of nurses and healthcare assistants.

Provide guidance, support, and ensure staff adhere to clinical guidelines.

Manage their development through training.

Clinical Governance and Quality Assurance:

Ensure compliance with regulatory requirements, hospital policies, and safety protocols.

Monitor clinical outcomes and quality indicators, and implement improvement plans where necessary.

Participate in clinical audits and ensure action plans are carried out based on audit findings.

Implement infection control practices and ensure adherence to hygiene standards.

Patient and Family Liaison:

Act as a point of contact for patients and their families, addressing their concerns and needs.

Provide emotional support to patients and their families during challenging times.

Ensure that patient dignity, confidentiality, and rights are respected at all times.

Manage the resources of the clinical unit, including equipment, medications, and consumables.

Work within a budget and contribute to cost-saving initiatives without compromising care quality.

Clinical Education and Professional Development:

Stay updated with advancements in nursing practices and healthcare technology.

Support and promote continuous professional development within the team.

Act as a mentor for newly qualified nurses and nursing students.

Emergency and Critical Situations:

Take the lead in managing emergencies and critical incidents in the clinical setting.

Coordinate with multidisciplinary teams to ensure swift and effective patient care in urgent situations.

Health and Safety:

Ensure the safety and well-being of both staff and patients by adhering to health and safety regulations.

Promote a culture of safety, identifying risks and taking proactive measures to prevent incidents.

Key Qualifications:

Registered Nurse (RN) with a valid practicing license.

Postgraduate Qualification in Nursing or a related clinical specialty (preferred).

At least 2-5 years of nursing experience, with some in a leadership or management role.

Strong clinical skills and knowledge in relevant specialty areas.

Leadership and Communication Skills to manage teams and liaise with multidisciplinary professionals.

Skills and Attributes:

Excellent organizational and time-management skills.

Compassionate and empathetic, with a strong patient-centred approach.

Ability to manage stressful situations and make decisions under pressure.

High attention to detail and accuracy in patient care documentation.

The role of a Clinical Sister is highly dynamic and central to the effective functioning of healthcare

teams. It requires strong clinical expertise, leadership abilities, and a compassionate approach to patient care.

Registered Nurse (adult) on part 1 of the NMC register Valid UK NMC PIN

Desirable
