J19 cares for patients aged 75 and over who are admitted directly from the Emergency Department. The ward is responsible for initiating treatment and delivering early care. Patients typically remain on J19 for a short period usually no more than a few days before being discharged or transferred to a longer-stay ward. This high patient turnover makes J19 a busy and fast-paced unit, supported by a hardworking and collaborative team that takes pride in delivering high-quality patient care.

We are immensely proud of the standard of care we provide and are seeking a highly skilled Sister/Charge Nurse to join our team, caring for our older adult population in Leeds. We are looking for an enthusiastic and dynamic individual who can empower and lead a team while maintaining a strong patient-centred focus. The successful candidate will coordinate clinical shifts, support the Band 7 in staff management, and contribute to the implementation of continuous improvement initiatives.

J19 is a diverse, hardworking, and supportive team within the Specialty and Integrated Medicine (SIM) CSU. We are passionate about delivering high-quality, patient-centred care. As an Acute Elderly Admissions Ward, J19 cares for patients aged 75 and over who are admitted directly from the Emergency Department. The ward is responsible not only for initiating required treatment, but also for ensuring patients have the best possible start to their hospital admission.

Comprehensive nursing assessments undertaken at the beginning of each patients journey form the foundation of their ongoing care. The nursing and care support worker team are committed to ensuring these assessments are thorough and accurate, while also making patients feel safe, supported, and well cared for. The J19 team prides itself on being inclusive and supportive, providing a welcoming environment for all new starters.

What you will bring to the role Acts in accordance with the Nursing & Midwifery Council (NMC) Code of Conduct.

Communicate with patients and carers to ensure they have access to appropriate information.

Maintain and deliver compassionate patient focused care in accordance with LTHT guidelines, policies, values and behaviours.

Provide assessment, planning, implementation and evaluation of nursing care for patients, ensuring documentation standards are upheld in accordance with NMC and LTHT guidelines, whether written or computerised.

Promote and maintain positive relationships between all staff involved in the care of patients.

Promote evidence based practice.

Participate in the audit and evaluation of clinical practice in the interests of promoting and improving quality care through action on findings.

Undertake duties on other wards or departments as and when required by service demands or patient need.

Act in such a way that safeguards the health and wellbeing of children and vulnerable adults at all times. Is familiar with and adheres to, the LTHT safeguarding policies.

Contribute to the promotion of a positive clinical learning environment.

Keep up to date with developments, advances and research in individualised patient care.

Participate in research and or education programmes relating to professional nursing issues and practice.

In the absence of the Senior Charge Nurse take charge of the ward or department and co-ordinate ward or department activities.
