Job summary

The post holder will be a member of the Bracknell Community Mental Health Team/Memory Clinic for Older People and as such will be mainly based at Church Hill House in Bracknell and work within its hours of operation as set out in its operational policy.

Shifts/Hours: Current working pattern is Monday to Friday 09:00am - 19:00pm (Shifts are: 09:00am - 17:00pm or 11:00am - 19:00pm and Saturday, Sunday and Bank Holidays: 09:00am - 15:00pm).

We are looking to recruit an Assistant Practitioner who enjoys working with older people to help the multi disciplinary team deliver a "needs lead service". As a member of the multi-disciplinary team your role will be varied, you will be responsible, under the guidance of a qualified professional, for delivering physical and mental health care interventions, to undertake physical health observations and report results to senior staff, including vital signs, monitoring blood pressure and pulse, taking ECG's, conducting the Antecedent Behaviour Consequence (ABC) charts, participate in mental health initial assessments, assessing for and participating in therapeutic group activities. The post holder would be expected to hold a caseload of patients, which would require supporting patients with mental health needs, under supervision.

This is a hybrid role, with approximately 60% of time office-based and 40% allocated to domiciliary visits and administrative tasks.

Main duties of the job

The post holder will also offer support to carers, and in collaboration with carers complete carers assessments, and maintain good liaison with GP practices, local care homes and other relevant agencies.

To have responsibility for a case load as specified by the Clinical Team Leader, in conjunction with Community Psychiatric nurses. To assess service user needs, plan effectively, monitor and evaluate interventions using evidence-based practice which is relevant to individual needs and within a framework of research and audit. To ensure the implementation of Trust and national policies regarding CPA and risk strategies, and in accordance with the current legislation To maintain comprehensive case records in line with Trust requirements. The role is developing, and additional duties may be added, full training will be provided to support the candidate to complete these duties.

Person Specification

Education/Qualifications/Training Essential

Good IT Skills including Word, Excel and PowerPoint. Basic knowledge of physical health conditions or willingness to undertake training.

Desirable

Good general education including GCSE Maths and English or equivalent. NVQ 3 in Health and Social Care or equivalent experience.

Continuous Professional Development Essential

Willingness to complete further training to enhance the role.

Previous Experience Essential

2 years experience of either a mental health or physical healthcare setting

Knowledge, Skills & Abilities Essential

Effective written and verbal communication skills including report writing. Able to engage respectfully with a range of different people including patients, carers and staff. Demonstrates insight into the patient's home environment.
