Job summary
The Advanced Clinical Practitioner / Paramedic Practitioner / Prescribing Pharmacist will provide autonomous clinical assessment, treatment, prescribing where within scope, medicines optimisation, escalation planning and advice to nursing teams for adult inpatients residing in Worcestershire Community Hospitals during out-of-hours periods.

About us
This service is expected to provide out-of-hours clinical cover to adult inpatients residing in local community hospitals and other local sites. The service supports timely clinical advice, assessment, prescribing, treatment planning, escalation decisions, end-of-life care and safe handover during evenings, overnight periods, weekends and bank holidays. The service covers patients across multiple community hospital sites according to clinical need.
Base/Location
The role will involve clinical review, advice and face-to-face visits across sites as clinically required.
Hours of Work
The service operates during the following out-of-hours periods:
Monday to Thursday: 18:3008:00 Friday 18:00 to Monday 08:00 Bank and public holidays: 24-hour cover

The postholder will work within their professional scope of practice and regulatory framework, supporting safe clinical care for patients receiving rehabilitation, intermediate care, sub-acute care, palliative care and step-up care.
The role may be undertaken by an appropriately qualified and experienced clinician registered with the relevant professional regulator, such as the NMC, HCPC or GPhC, with prescribing capability where required by the service model. The service specification recognises clinicians may be registered with GMC or an equivalent core designated regulatory body such as NMC or HCPC.

Qualifications Essential
Be registered with the appropriate professional regulator, for example NMC, HCPC or GPhC, according to professional background. Hold an advanced clinical practice, paramedic practitioner or prescribing pharmacist qualification appropriate to the role. Hold independent prescribing qualification where prescribing duties are required. Hold professional indemnity insurance appropriate to the role. Have appropriate certificates confirming eligibility to work in primary care or relevant clinical services. Have the right to work in England. Have an enhanced DBS check. Meet NHS Employment Check Standards. Communicate effectively in English with patients, carers and colleagues. Participate in annual appraisal and personal development planning. Complete onboarding, including training on RIO and Bettermeds / ePMA systems. Maintain annual Basic Life Support training and provide evidence when requested.
Person Specification Essential
Current registration with relevant professional regulator. Advanced assessment and clinical decision-making skills. Experience of urgent care, primary care, community services, frailty, emergency care, intermediate care or similar settings. Ability to assess acutely unwell adults and identify clinical deterioration. Ability to work autonomously within scope of practice. Independent prescribing qualification if undertaking prescribing responsibilities. Understanding of safe medicines management, antimicrobial stewardship and local prescribing governance. Ability to use electronic clinical records and prescribing systems. Excellent communication skills with patients, relatives, carers and multidisciplinary teams. Ability to travel between Worcestershire sites as required. Commitment to equality, confidentiality, clinical governance and patient safety.
Desirable
Masters level advanced clinical practice qualification or equivalent portfolio evidence. Experience working in community hospitals or inpatient rehabilitation settings. Experience of end-of-life care and palliative deterioration. Experience using RIO, Bettermeds or similar NHS clinical systems. Familiarity with Worcestershire community pathways and local formulary arrangements. Experience contributing to audit, clinical governance or quality improvement.
