Main duties of the job Key responsibilities...
Undertake personcentred assessments to identify housing adaptations that promote safety, independence and longterm wellbeing Work collaboratively with service users, colleagues and partners to develop and progress DFG solutions Contribute specialist OT knowledge to multidisciplinary meetings, case discussions and service development Produce clear, evidencebased reports and recommendations to support DFG applications and adaptation specifications Maintain accurate electronic records in line with professional and organisational standards Engage fully in supervision, appraisal and ongoing professional development Practice in accordance with council policies and the HCPC Code of Conduct and Standards of ProficiencyBracknell Forest Council is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults.
