Job summary
An exciting opportunity has arisen for a motivated and organised Care Coordinator to join the Congleton and Holmes Chapel (CHOC) Care Community on an initial 12-month fixed-term basis.
The postholder will play a key role in supporting the delivery of proactive, coordinated and patient-centred care as part of the Better Care Fund (BCF) workstream. This particularly focuses on patients with complex needs, including long-term conditions, frailty, dementia and palliative care needs.

This vacancy will be a 12-month fixed-term contract
The post is offered at up to 22.5 hours per week, typically worked across Monday to Thursday. There is flexibility in the working pattern, which will be discussed and agreed with the successful candidate.
The role will require travel across CHOC PCN GP Practices in both Congleton and Holmes Chapel.

Main duties of the job
The Care Coordinator will work closely with the Care Community Support Manager, BCF GP and GP practices to support patient care and service delivery of the BCF programme across the CHOC Care Community.
Key responsibilities include:
Supporting the identification and engagement of patients for the BCF programme Coordinating clinics, appointments, and clinician schedules Managing patient lists and maintaining accurate records using EMIS Supporting personalised care and planning Collect and document patient background information including social circumstances, lifestyle factors, personal goals and priorities. Monitoring patient progress and coordinating follow-up care Organising and supporting multidisciplinary team (MDT) meetings (frailty & dementia) Supporting data collection, reporting, and service monitoring Communicating effectively with patients, carers, and healthcare professionals

Person Specification
Qualifications Essential
Educated to NVQ Level 3 or equivalent experience in health or social care Evidence of relevant administrative or coordination experience
Desirable
Qualification in health, social care or care co-ordination Training in personalised care or care navigation
Experience Essential
Experience working in a healthcare, primary care or NHS environment Experience coordinating services or managing patient pathways Experience of administrative work including scheduling and record keeping Experience using clinical systems such as EMIS or similar
Desirable
Experience supporting multidisciplinary team (MDT) meetings Experience working with patients with long-term conditions, frailty or complex needs Experience of clinical coding or healthcare data management
Skills and Knowledge Essential
