Job summary
We are looking for a part time clinical pharmacist to join our existing team, ideally for approximately 30 hours per week. This is a really interesting opportunity for a breadth of primary care experience as an Independent Prescriber, both within the Primary Care Network and also Hillfoot Surgery. This means you would get the opportunity to work across multiple primary care contracts all within one role (PCN DES, QoF, GPOP, IIF). The successful applicant will be employed by WLPCN Ltd but partially deployed to Hillfoot Surgery
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation across the PCN. This will include structured medication reviews with direct patient contact and may include contributing to:
Management of long-term conditions
Minor illness
Reviewing systems for safer prescribing
Action acute prescription requests
Addressing both the public health and social care needs of patients
Contributing to achievement of QOF and locally commissioned quality improvement schemes.
Undertaking clinical audit
Developing, promoting and implementing the high quality, evidence based and cost-effective use of medicines in primary care in a way that maximises the benefits of medicines to patients and service users and which minimises risks (clinical, legal and financial).
