Job summary
We are seeking an enthusiastic Individual to work within Southampton Integrated COPD Team.
The role supports the delivery of Pulmonary Rehabilitation classes and support complex respiratory disease community clinics.
The successful candidate will be involved in working with patients with long term lung conditions, helping to maximise their function and support long term behaviour change.
This is a rewarding role requiring an individual with good people, organisational and administration skills. It is essential you can work in a team and be flexible in your approach. Candidates with experience of working with specialist groups of clients in a group exercise environment are encouraged to apply however not essential.
You will be working across sites in Southampton City. The post holder is required to be a car driver.
The Service currently runs between the hours of 08.00-18.00 and your work pattern will fall within those times. There is no weekend working in this post at present.
The post is for 37.5 hours a week.

Main duties of the job
This role will be split between Pulmonary Rehabilitation (PR) and Specialist Respiratory Community Clinics, delivered within the community across various locations in Southampton City. You will be a key member of the Integrated COPD team to support effective delivery of community COPD care.
The role involves supporting assessments and delivery of PR classes (both exercise and educational delivery) alongside the clinical lead for the programme, ensuring patients are comfortable, confident with the exercises and feeling part of a group. You will provide 1:1 assistance to patients, assisting with patient related admin and organisation of equipment, including ordering of supplies. You will be contributing to the delivery of the health promotion agenda and collaborating with third party providers to support patients in long-term behaviour change.
The role in Specialist Respiratory Clinic will be supporting Clinicians/Consultants with clinical assessments by gathering clinical information, conducting inhaler technique reviews, completion of health-related questionnaires and provide self-management advice within scope of competencies.

Person Specification
Qualifications Essential
*NVQ Level 3 in appropriate subject field e.g., Health and Social Care or Diagnostic or Therapeutic Support. OR *Exercise Qualification to REPS/CIMSPA Level 3 in Special Populations e.g., GP Referral, Falls, Cardiac, Pulmonary etc. *Holds UK driving licence suitable for business insurance in the UK and access to car for work purposes. *Demonstrates commitment to learning via personal development file. *Completed care certificate, in the process or willingness to work towards the care certificate.
Desirable
*Completed Care Certificate *Health related degree/further education.
Experience Essential
*IT literate and to feel comfortable with using a laptop due to working within community settings. *Ability to carry out assigned tasks effectively in a busy environment. *Completion and maintenance of statutory & mandatory learning activities. *Evidence of managing own workload / clients. *Effective written and verbal communication skills/literacy/ numeracy skills *Good observation skills and recognition of the deteriorating patient and importance of reporting/ recording.
Desirable
*Experience of working with IT systems. E.g., Microsoft Word, Excel, Systm1, RIO, EMIS. *Evidence of working with people with complex and or progressive conditions. *Evidence or working in hospital or social care setting. *Evidence of contributing to the development of others. *Evidence of having worked as part of a Multidisciplinary team or with social agencies and charities. *Experience of working in Pulmonary Rehabilitation.
