About this role

Job summary

The main purpose of this job is to:

Facilitate access, to medical and other health and social/well-being related appointments for patients who typically experience barriers when accessing healthcare services.

Act as a focal point for signposting and communication between patients, doctors, nurses, other members of CAVHIS and external services.

Undertake various administration duties associated with patient care and the efficient running of CAVHIS.

An exciting opportunity has arisen to be part of the Cardiff and Vale Health Inclusion Service (CAVHIS), which aims to deliver an equitable and high standard of care to vulnerable groups in Cardiff and the Vale. We are keen to recruit someone who shares our passion for this client group.

The successful applicant will have excellent communication skills and will work well within a team. The ability to cope in stressful and demanding situations is highly desirable, although support will be given.

Person Specification

Qualifications and Knowledge Essential

Educated to GCSE Standard or equivalent to NVQ Level 3 in Administration or similar

Desirable

ECDL

Experience Essential

Previous experience of working with groups who face barriers when accessing mainstream services (for example, people experiencing homelessness, asylum seekers etc). Previous experience of working with Microsoft Office Packages. Knowledge of Patient/Case Management Systems

Desirable

Previous experience of working in the NHS or similar environment Experience of minute taking Knowledge of UHB patient management systems (e.g., Paris, E-MIS, Welsh Clinical Portal etc.).

Skills and Attributes Essential

Excellent communication skills, including call handling Problem solving skills Excellent IT skills, including typing and keyboard skills Ability to work in a fast paced and changing environment. Demonstrate effective organisational skills and able to prioritise workload Manage own workload independently Good understanding of patient confidentiality, GDPR and Caldicott principles Pro-active approach to patient care Patient and empathetic in distressing situations Flexible attitude to work.

Desirable

Ability to speak Welsh/any other languages. RSA Typing Understanding of de-escalation skills. Understanding of legislation relating to hard to reach groups (e.g., asylum seeker/homeless law). Understanding of UHB/Welsh Government targets. Understanding of patient pathways. Committed to providing a quality service. Ability to travel when required.
