Job SummaryIncumbent is a clinical pharmacist at this Fayetteville VAMC. Rotation through Inpatient and Outpatient sections is required. Must possess a professional knowledge of a wide range of concepts, principles, and practices of pharmacy science. Tour of duty for pharmacist will include rotation of shifts as well as weekend and Holiday coverage.

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). c. Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-12 Clinical Pharmacist (Full Performance Level) (a) Experience or Education. In addition to the basic requirements, candidates must meet one of the following: 1. 1 year of experience equivalent to the next lower grade level, or 2. CLINICAL (1) Incumbent provides consultation to outpatients and family members regarding medications as to their use, side effects, and precautions. Reviews polyprescriptions for drug interactions, potential synergism, and consults with prescriber(s) on course of action. (2) The incumbent furnishes the medical staff and nursing staff advice or information regarding drug usage, availability, storage, dosage, and incompatibilities etc. (3) Screens patient profiles for correct medication, dosage, drug/drug or drug/food interactions before dispensing when rotating through the inpatient section. (4) Monitors/doses patient I.V. therapy including Total Parenteral Nutrition (TPN) as allowed by protocol and/or policy. (5) Receives and institutes verbal orders from approved Practitioners. (6) Doses medications for infusion clinic and acute care as allowed by protocol or scope of practice as applicable. (7) Provides refill extensions of prescriptions and partial supplies of noncontrolled maintenance medications. (8) Assist in formulary management, including restricted medication/nonformulary evaluation and therapeutic substitution. (9) Assist in medical emergencies and medication use evaluations (10) Solve medical problems as related to medication therapy and oversee technical staff in all aspects of medication distribution. (11) Reviews and accurately completes pending prescriptions for transmitting to and filling at VA mail order facility (CMOP). Minimizes errors that contribute to CMOP rejections of transmitted orders. As workload dictates and allows, may work remotely in providing services. (12) Interprets lab data and other diagnostic tests as appropriate to assess drug therapy and outcomes. (13) Other duties as assigned. B. DISPENSING (1) Incumbent evaluates physician's orders as to appropriateness based on patient medication profiles; inputs information into the computer system as assigned; dispenses medication in accordance with dosage form requirements (outpatient, unit dose, I.V. admixtures). (2) Provides oversight, checks, guides, and advises technicians on the receipt, interpretation, and compounding of prescriptions. (3) Is responsible for the preparation and dispensing of Antineoplastic agents in both the inpatient and outpatient setting. This may involve oversight and review of technicians preparing the medication. (4) Accurately performs pharmacist duties in the medication dispensing and delivery process. This includes accurate receipt, interpretation, entry, and review of orders. Must accurately check prescriptions prior to patient dispensing. Must perform clinical duties accurately within appropriate standards of care while performing dispensing functions.
