This is an exciting time to work in Cambridgeshire South as we accelerate our Neighbourhood Health Programme - an ambitious, system-wide effort to redesign how care and support is delivered across our Place.
The Integrated Neighbourhoods Project Manager will provide transformation support and project management to help develop IntegratedNeighbourhoods and build relationships between organisations.
We are looking for a Neighbourhood Project Manager to join our small innovative team for a up to a12-month period; offered as a secondment or fixed term contract.This opportunity would be perfect for anyone keen to learn more about and support thedevelopment of Neighbourhoods across Cambridgeshire South area and specifically in theEastCambridgeshirelocality.
Your role will be to work collaboratively with partners and Communities to align services and support,continue to embed our relational way ofworking and support the ongoing development of our Neighbourhood Teams, to recognise opportunities and identify how they can improve care and support and then helpto deliver this change.
You will act as a relationship builder, a connector, a problemsolverand a cheer leader, always promoting the neighbourhood ways of working at a time of significant pressures and challenges. You will provide transformation and project managementexpertiseto make positive changes, buildstrong and sustainable foundations for Neighbourhood Teams and to tackle inequalities.
Main duties of the job
Provide transformation support and project management for the Integrated Neighbourhoods programme ofwork. Review population health data and support public engagement todeterminelocal needs andhowcitizenscanshape theimprovement oftheirhealthandcaresupport. Be responsible forsupporting the service redesign and delivery of specific projects within the Integrated Neighbourhoodparticularlyaroundlong-termconditions andhealthinequalities. Workclosely to build relationships and bring partners and communitiestogether tocreate a sharedvision, priorities andmakepositive changes to the waywecollaborativelysupport ourcommunity'shealth &wellbeing. Work closely withClinical leaders,PrimaryCare,the acute sector, community services, health commissioners,local leaders, local authorities, voluntary and community organisations, and the public to support the development of the servicedesign. You may line manage project support officers and will work closelyand collaborativelywith the other Integrated Neighbourhood Project and Programme Managers.

The Integrated Neighbourhood programme aims to co-develop a Neighbourhood Health Service that supports people to stay well in their own communities. Working in partnership with Primary Care, Community Services, local authorities, the voluntary sector, we are developing new models of support and care to deliver against the national Neighbourhood Health Framework.

We are particularly interested in anyone with experiencein data collection,analysisandevaluation-measuring what matters most to people, communities and our Neighbourhood workforce,and co-designing services around this,is akey focusof the role in East Cambridgeshire.We are looking for someone who enjoys working with people and bringing the best out of them, a person with a confidence to work autonomously and opportunistically in a constantly evolving environment.
As this is a short-term position for up to 37.5 hours per week, secondments (please seek your managers permission before applying) and flexible working / reduced hours proposals are welcome.
