Job summary
Brynmawr District Nurse Team is fundamental in the delivery of the health boards' clinical futures model in providing care closer to home enabling early discharge and preventing hospital admissions.
We primarily deliver holistic care to patients within their own homes, residential homes, day centres and occasionally support staff and residents of nursing homes to meet the increasing complexities in the health needs of our patient population.
Assessment, planning and delivery of high standards of care are achieved through effective collaboration and partnership working.
Main duties of the job
We primarily care for patients and their families within their own homes and residential homes and nursing care also occasionally being supported in other care setting s such as day centres and nursing homes. The nurses of our community team feel privileged to support patients in the setting of their choice.
Our district nursing team have an exciting opportunity for a registered nurse to join our community team and support our community based service provision.
You will have opportunity to develop your clinical and interpersonal skills with continuing professional development opportunities to prepare you with the skills to meet the evolving autonomous role of the community nurse through engagement of a robust induction programme and further developmental training.
