About this role

Job summary

This role is for a Band 5 Mental Health Nurse to be part of the West Wiltshire Community Health Team based at Shearwater Lodge in Warminster, Wiltshire. We have two other hubs, one in Trowbridge and one in Devizes.

This role is focused on providing appropriate, effective interventions and treatments to people with severe and enduring mental health needs, and their carers (including friends and relatives) and their supporters in the community, enabling and assisting them to meet daily health, social care and wellbeing needs, in line with personal recovery goals, and facilitating engagement with mainstream services.

The role involves being responsible for Personalised Care and Support Planning through the AWP Your Team-Your Conversation-Your Plan approach, including taking on the role of "keyworker" at times and completing Safety Assessments. There may also be requirements to support the delivery of Social Care Assessments and reviews where required.

Main duties of the job

The role involves providing profession specific interventions to people and bringing professional expertise into the Multi-Disciplinary Team. In addition, there may be a need to provide non-profession specific, defined, personalised interventions that are agreed through Support Conversations. These might include a range of possible generic, evidence based therapeutic interventions where staff are trained to deliver these interventions.

To work as part of the MDT and actively participate in weekly MDTs.

Undertake additional training relevant to the role.

Administer depot injections to Service users.

Person Specification

Diploma level/Degree in nursing Current relevant registration with the NMC commitment to CPD Highly developed verbal communication skills, able to engage effectively with people at all levels even when a more assertive approach is needed. Substantially developed IT skills including competent use of Microsoft Office, and the use of electronic record keeping systems valid Driving licence Access to a vehicle for work

Desirable

Is able to contribute to the delivery of a range of possible key therapeutic interventions. Demonstrates a good understanding of recovery principles and the role of secondary mental health services in the delivery of care. Demonstrates significant experience of assessing risk and developing risk management strategies.
