We are looking for a Community Health and Wellbeing Worker (CHWW) to work in a team of four. You will be based in Littlemore and employed by SEOxHA PCN, an organisation consisting of Donnington, The Leys and Temple Cowley Health Centres.
Main duties of the job
As a CHWW, you will be in a patient-facing role, engaging with members of the community to identify ways to improve the health and wellbeing of all people, from the young to the old, connecting them to access appropriate support and community resources. This role bridges health and social services and the community. CHWW are passionate role models who will provide advice and signposting to everyone within their communities, enabling them to make positive choices around health and well-being. Youll meet people face to face, helping them to find local health services, offering to help them make appointments or signpost them to local services to support their needs.

The SEOxHA Network delivers health and wellbeing services to a population of 33,000. The local community is a rich blend of cultures and diverse backgrounds.
This is a unique opportunity to develop the role within the Primary Care Network (PCN) and work with other team members to develop the PCN as a whole.
There are clinical pharmacists and a pharmacy technician, social prescribing link workers, and learning disability & care home patient coordinators in the PCN.

Alongside participating in evaluation activities within a project team, you will play a significant role in increasing health promotion and wellbeing through a range of activities, including:
Outreach assessments, Signposting, Education, Group/Community events

Dont worry about having previous NHS experience; full training will be provided as you help us deliver support to the local communities in Littlemore.
Hours: 20 hours per week Monday, Tuesday 9.00am to 3.30pm and Wednesday 9.00am to 5.30pm (30 minute unpaid break), with occasional evening and weekend hours required. Fixed term contract.
