Main duties of the job
Assess, diagnose, plan, implement and evaluate treatment, interventions and care for patients presenting at the practice.
Clinically examine and assess patient needs and plan clinical care accordingly.
Assess, diagnose, plan, implement and evaluate interventions/treatments for patient with complex needs.
Prescribe and review medication in accordance with practice and national protocols.
Undertake home visits
Undertake telephone advice calls.
Maintain accurate record keeping with Systm1.
Request and interpret tests appropriate to patients needs.
Where appropriate make referrals to other health professionals in primary and secondary care.

Job Summary
The post holder will be able to perform as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions.
Main Duties and Responsibilities:
Assess, diagnose, plan, implement and evaluate treatment, interventions and care for patients presenting at the practice.
Clinically examine and assess patient needs and plan clinical care accordingly.
Assess, diagnose, plan, implement and evaluate interventions/treatments for patient with complex needs.
Prescribe and review medication in accordance with practice and national protocols.

Person Specification
Qualifications Essential
Registered Professional. Non-medical prescriber qualification. Evidence of advanced qualification
Experience Essential
Minimum 1 year post registration experience Experience in management of patient with complex and long term needs Proven ability to evaluate safety and effectiveness of their own clinical practice Ability to work in a team Excellent communication skills both written and verbal
Desirable
Previous experience of working in a GP practice or primary care
